#ChiefResident blog #NoonConference #meded @jungleland Heparin induced thrombocytopenia (HIT), diagnostic test EBM! imrespdx.com/imrespdx-blog/…

@jungleland One way to look at true thrombocytopenia is underproduction vs. sequestration vs. destruction #NoonConference #meded

@jungleland HIT is largely a clinical diagnosis & empiric txn should be started in a patient with very high pretest probability #Meded #NoonConfernece

@jungleland When applying a dx test, we should use a Bayesian approach: derive a pretest prob & use likelihood ratios to generate a posttest probability

@jungleland However, if we have extremely high pretest prob we should consider not even ordering the diagnostic test if it would not change management

@jungleland See this superb blog post on how to apply likelihood ratios and myths about sensitivity and specificity @emcrit emcrit.org/pulmcrit/mythb…

@jungleland lesson here--tests all have limitations, a negative anti-PF4 assay doesn't 100% exclude HIT #meded #NoonConference

@jungleland check out Dr. Bien's recent #ACPOR17 poster from this case! Available on our virtual poster session #meded ohsu.edu/xd/education/s…
