So, deliberately losing records of families, transporting people thousands of miles, and using inter-agency lack of communication to ensure children *can't* be reunited with their parents? Yup, that's exactly what happened.

If you take 3,000 children on a *field trip* and say "well, asking us to get them back to their parents within a month is just extreme!" people will (a) think you're crazy or lying and (b) haul you off. Why is it justified here?
Keeping track of these children would have been the easiest thing imaginable — I've solved more complicated logistics problems with a single spreadsheet. There is no "secret hard part" to doing this, just a deliberate decision not to do the obvious.
If you're thinking that tracking kids as they're moved from holding facility to holding facility, orphanage to foster family is complicated, this was literally solved at scale decades ago. Transfers are recorded in the middle column.
