One of the reasons elite academic, professional and medical settings shy away from the full acknowledgement of disabled personhood is that to do so would force many to acknowledge that friends and colleagues are moral (and at times, actual) criminals.
If you are in certain settings, the belief that all disabled people, including people with intellectual disabilities, are full persons forces acknowledgement that people you know, work with and respect have done things that, in a just world, would merit serious punishment.
And so a mental process takes place:

1) I do not work with people who enable torture & murder

2) Some of my colleagues enable things that would be considered torture & murder if they were done to people.

3) The disabled people these things are done to must not then be people.
To preserve (1), which is crucial to the self-image of so many doctors, ethicists and others, requires ignorance of (2) or acceptance of (3).
Anything else would require either accepting that (1) is not true, which threatens the ego far too much, or breaking off relationships and condemning colleagues in terms incompatible with professional norms.
If we are to break this logjam and secure unqualified recognition of disabled personhood in these settings, one of two things are necessary:
a) Aggressive efforts to promote the shunning of those who have complicity in the murder and torture of the disabled.
b) Teaching our society's elites that people who are outwardly pleasant, helpful and even intellectually charming to them may still be murderers and torturers, or enablers of the same.
I am not sure which is more difficult. One breaks the bounds of professional civility, the other shatters self-image and ego.
This is the fundamental challenge facing those who would preach disabled personhood in academia, medicine and many other crucial spheres where it is now held in doubt.
