So reviewing all of the chronic conditions that puts someone at risk for complications & mortality w/ COVID-19 a little overwhelming this a.m. Trying to provide pop health data to support RN Care Managers provide virtual support to keep our patients healthy & strong #NurseTwitter
1st order of business identifying all those w/existing pulmonary disease, neb med refills within last 90 days & potential for MDI in near future. Concerned about albuterol shortage given hospital 8-10 puff treatment in lieu of nebulizer given risk of transmission. #NurseTwitter
2nd order of business reaching out to pulmonary patients to provide edu abt COVID-19 prevention, proper use MDI & tracking doses, & nebulizer safety if they must do TX in home. Educate about preventing condition exacerbation to reduce need for ER or in-patient care. #NurseTwitter
3rd moved on to other chronic conditions putting them at risk for COVID-19 complications like DM, HTN, CKD, CAD/MI/HF, CA, HIV, Obesity etc. Combined w/ those at risk for ARDs like Age, HX of ETOH use & tobacco use & high risk surgery. #NurseTwitter
Messaging & support will be the same regardless of the condition: 1. COVID-19 prevention, 2. Chronic condition management - exacerbation prevention, 3. Remote monitoring of bio-metrics, 4. Self-management - problem solving, decision making, action planning, coping #NurseTwitter
Barriers to self-care will remain & provide additional challenge to staff supporting our already vulnerable & at-risk population w/ multiple SDOH like housing insecurity, food insecurity, financial insecurity & inconsistent access to communication technology. #NurseTwitter
Other challenge includes supporting pop w/ mental health & substance use disorders. Wonder how others are supporting DOT & Med Admin programs while trying to eliminate visits to site for routine care? Researching #Telehealth methods to support #MedicationAdherence #NurseTwitter
Trying my very best to put together population health standards to keep our vulnerable & risk at-home, healthy & reduce risk of ER & in-patient utilization. #shoutoutSaturday to all the ER & in-patient #Nurses. Doing all that I can to support you & reduce #PPE use. #NurseTwitter
