Considerations for meds:
Pt education on lovenox/insulin/nebs/inhlaers
Can they afford meds? Refills?
Confirm pharmacy & send 1mo supply (non-opiates)
Pill pack? Let the pharmacy know so they can prep
Going to SNF/IRF on Fri/wknd? Send 3 day supply of controlled substances 3/
Any special services? We are talking about all things home health: OP O2 , BiPAP/CPAP, IV antibiotics. Your case management point person (or social work) will be vital in this process. Lastly, be sure to notify PCP and list on discharge paperwork. #POPCoRNtweetorial 4/
Let’s not forget about last minute procedures. Do they need a foley/drain removed or exchanged? Trach change/supplies? Or PICC line for long term access? Thinking about this early can help prevent an extended hospital stay #POPCoRNtweetorial 5/
Proper follow-up is KEY
Appt w/ PCP (+subspecialists) in 1-2 wks
Telemedicine visits may require coordination
Incidental findings during their stay? Order follow-up scans per Rad recs (esp. if need in <2mo)

THE discharge:
Msg PCP on important findings (if not automatic thru EMR)
Med rec: remove hospital meds & start appropriate home meds
Take the time to education pt/family on which meds to START, STOP, & HOLD until PCP visit
