why does the web link say "surprise medical bill"? This was clearly an in-network claim, which, by default, can never be SMB? 2/x 
The plan appears to be a high deductible plan, per the EOB. That's how HDHC plans work - even in-network, these plans have, well, a high deductible. Not sure how we got from that to the surprise medical billing part. 3/x
much of the journalistic work on #SMB and legislative corrections to prevent it focus heavily on physician charges. On this claim, of $3500 charged, $198 was physician charges. Big gap there. 4/x
To the claims processing. Absolutely correct that cost sharing is expected to be waived for COVID related testing and care. Both private and public payors included. @LorenAdler pointed out some bad behavior with self insured commercial plans last week. 5/x
There are codes and modifiers that can be used to designate a COVID patient. Would love to see the I-10 of how this claim was initially coded and "corrected" to the point @UHC honored their obligation. Open for discussion. Overall, happy this was resolved. final/x
