Ischaemic Heart Disease (stable)

✔️ Antiplatelets - check DI
✔️ Tele clinic - support & address concerns.
✔️ Encourage patients to seek help if symptomatic

Atrial Fibrillation / VTE (stable)

✔️ Consider switching warfarin to NOAC where appropriate to reduce need for INR monitoring.
