Lipid management (stable)
✔️ Carry on as usual
✔️ Tele Clinic to support patients and address concerns.
✔️ Check interactions with medicines if new therapies were prescribed. (DI)

Heart Failure (stable)
✔️ Carry on as normal (check DI)
✔️ ACEI / ARBs / Entresto carry on as normal.
✔️ Tele clinics to support patients and address concern.
✔️ Encourage patients to report / seek help if deterioration of symptoms
✔️ Links with HF teams

Ischaemic Heart Disease (stable)
✔️ Carry on as normal
✔️ ACEI / ARBs continue.
✔️ Aspirin not a concern (NSAID) at lower dose.
✔️ Antiplatelets - check DI
✔️ Tele clinic - support & address concerns.
✔️ Encourage patients to seek help if symptomatic
✔️ Links with CR teams

Atrial Fibrillation / VTE (stable)
✔️ As usual (check for DI)
✔️ Consider switching warfarin to NOAC where appropriate to reduce need for INR monitoring.
✔️ Tele clinic to support patients.
✔️ Link with Anticoagulation Team

Hypertension (stable)
✔️ Continue as usual
✔️ ACEI / ARBs continue.
✔️ Tele clinic to support patients and address concerns.
✔️ Blood pressure self monitoring advised. (relevant to all above conditions)

Other considerations
✔️ Support adherence
✔️ Home BP monitoring
✔️ Check drug interactions
✔️ Check new drugs (COVID-19) that can prolong QT.
