In this case, first a normal vaginal delivery at term 5 years ago, second a CS for transverse lie at 36 weeks in labor, 3 years ago. There is a history of spotting 2 weeks ago and now she has soaked her undergarment, and there is no history of abdominal pain.
On Ex:
The patient conscious, oriented, vitals are maintained, and mild pallor. The uterus is relaxed, 32 weeks in size, cephalic free-floating, fetal movements are present and fetal heart sounds heard. No active bleeding was seen at present.

What is placenta previa and it’s classifications ?
Placenta praevia is a placenta that is partially or wholly implanted into the lower uterine segment.
It is usually discovered in the last 6 months of pregnancy and sometimes in the first 3 months diagnosed by ultrasound.

المشيمة المنزاحة وانواعها:
حالة يكون فيها موقع المشيمة داخل الرحم غير طبيعي بحيث تكون قريبة او تغلق قناة عنق الرحم. المشيمة في حالتها الطبيعية يكون موقعها في اعلى جدار الرحم 🤰🏼
وتعتبر المشيمة المنزلحة احد الاسباب الرئيسيه لنزيف 🩸ماقبل الولادة.
حالة يكون فيها موقع المشيمة داخل الرحم غير طبيعي بحيث تكون قريبة او تغلق قناة عنق الرحم. المشيمة في حالتها الطبيعية يكون موقعها في اعلى جدار الرحم 🤰🏼
وتعتبر المشيمة المنزلحة احد الاسباب الرئيسيه لنزيف 🩸ماقبل الولادة.
عادة مايتم اكتشافها بالاشهر الستة الاخيرة من الحمل واحيانًا بالاشهر الثلاثه الاولى، يتم تشخيصها عن طريق الاشعة فوق الصوتية (Ultrasound).

3-النساء اقل من٢٠ سنة واكبر من 40 سنة
4-وجود ندوب على الرحم، نتيجة لجراحة سابقة،مثل جراحات الولادة القيصرية،وإزالة الأورام الليفية الرحمية، وتوسيع الرحم و كحته
5- التدخين.

2-Shock (depend on the amount of bleeding).

-The uterus is corresponding to the period of amenorrhoea, relaxed, and not tender.
-The fetal parts and (FHS) can be easily detected.

-صدمة shock تعتمد على كمية النزيف.
-باجراء الفحص على بطن الام (حجم الرحم يقابل فترة انقطاع الحيض )(لا يوجد الم في البطن ) (ويكون البطن مرتخي ومن السهل سماع ضربات قلب الجنين).

It can be in the form of conservative or active management depending on:
1-The severity of blood loss and whether it has stopped or is continuing
2-The condition of the mother and fetus
3-Gestational age
4-The onset of labor.

المضاعفات 🚫:
-النزيف. يمكن حدوث نزيف دم مهبلي شديد مهددًا للحياة خلال المخاض أو الولادة أو الساعات القليلة الأولى بعد الولادة.
-على الجنين:من الممكن ان يسبب (IUGR): حالة لايبلغ فيها نمو الجنين الحد الطبيعي، او الاجهاض وعدم اتمام الحمل.
ومن الممكن ان لا يكون هناك مضاعفات.
-النزيف. يمكن حدوث نزيف دم مهبلي شديد مهددًا للحياة خلال المخاض أو الولادة أو الساعات القليلة الأولى بعد الولادة.
