Every facility should have a roles and responsibilities matrix (R&R), that's how everybody knows what to do when they arrive to work. and That’s why they know whom to call when the shit hits the fan.
The R&R is also coupled with escalation matrix with multiple levels so when you call a supposedly responsible person, and they don't answer / react in a timely manner, the escalation matrix tells you whom to call next / how / and when / and how frequently
The “RACI" R&R for every silly granular task, lists the responsible (doing the job like the *smith*), accountable (approves, signs), consulted (experienced not responsible), and Informed (FYI). now it gets interesting >>>
The accountable is also *responsible* for drafting the R&R and is held *responsible* whenever a task lacks the RACI assignment!!! so basically in every operation you can immediately find someone to blame ... (and is definitely not the smith)
Success requires that there is only one person Accountable, which means "responsibility stops there." >>> now whenever a problem occurs the responsible / accountable should have a disaster recover plan (tested regularly) .. but this is for another thread!
