One of the most inspirational people on my Twitter timeline is @Lyndseydunn7.

Now talking here at the @GeriSoc #bgsconf on delirium and how we need to identify it and manage it in the real life situations we see every day.
It’s very impressive to see what Lyndsey and the @NHS_Lothian team have achieved with their programme.

#Delirium
#bgsconf
We know what the definition is for #delirium, yet the language used in clinical records is often poor. Well said, @Lyndseydunn7 !

#bgsconf
It’s about recognising a change and acknowledging that #delirium is a serious condition.

#bgsconf
Who can do the 4AT to assess for #delirium?

Everyone!

#bgsconf
What are we trying to achieve?

#delirium #bgsconf
Just ask the single question if you don’t have much time!

#delirium
#bgsconf
A great programme from @Lyndseydunn7 and her @NHS_Lothian colleagues to raise awareness of #delirium.

Well done, everyone! Note the QR codes!
A great success! Well done all!

Would you like a new dinner idea?

This is a new recipe for us, but was very tasty…

It’s going to be a thread, I think. 🧵 Slice of oven baked dinner with cheesy topping.
First, break up a cauliflower into florets…

Toss in olive oil, salt and pepper.
Pop it in the over for half an hour to roast.

Already looking yummy (if you like cauliflower, which I do!). Cauliflower in a dish.
Next, make your tomato sauce.

Garlic & onion, again in olive oil. I put in half a courgette (diced) because it had out stayed its welcome in the fridge.

Two tins of tomatoes and some fresh basil (thank you @sainsburys this week) and we’re almost ready… Image

I’m still reading @danielleofri’s book on communication…

We’re not very good at this in healthcare, are we?

And this my friends, is why I used to write these things down for my patients when I worked in the acute medical unit at the Royal Infirmary…
This sort of thing… it doesn’t take long to scribble a few words for someone, and their family can read it too when they arrive later in the day.

“Palliative care can benefit people with dementia”.

From last week’s ⁦@bmj_latest⁩, it’s good to read ⁦@kesleeman⁩ and ⁦@emelasml⁩ raising the issues of providing good care to people with dementia.

Core business for GPs & Geriatricians! bmj.com/content/380/bm…
It’s interesting to read the responses to the piece.

A lot of very good palliative care already happens in care homes up and down the country.

We need to respect that there’s a lot of expertise in these settings.

GPs and care home teams are doing great things in many places. Responses to article. Argui...
It’s not universal, and part of the answer may be to resource properly the care homes and primary care teams, rather than necessarily import “palliative care” expertise at the end of life?

Geriatricians, community nurses & GPs are very used to the inherent uncertainties faced.

The debate on whether we should add folic acid to food has been going on since I was a medical student.

In last week’s @bmj_latest, @munkeatlooi looks at some of the pros, cons and evidence.

Social inequalities play into this too.

Folic acid has been a hot topic for years.

We know that supplementation of this vitamin during pregnancy reduces the chances of having a baby with a neural tube defect (NTD) eg spina bifida, which can cause significant disabilities.

But not everyone takes it.
Quote:

“Women can get folic acid from their diet, but current diets are unlikely to meet recommended levels. This is because of low intakes of whole foods that contain folic acid, particularly among more disadvantaged groups that face systemic barriers to eating well…”

Some further slides from today’s #bgsconf, this time on Pharmacy support to care homes.

Spoiler: it is a bit variable! ImageImageImageImage
It’s particularly variable how many care homes each Pharmacy team is supporting…

#bgsconf ImageImageImage
So what are the roles undertaken by Pharmacy teams in #CareHomes?

Lots of different things!

A lovely presentation from Dr Pam Seenan on her team’s work with the Beatson Oncologists to improve care of older people with cancer.

Geriatric Oncology is a whole new sub specialty.

Call the COPS! (Cancer of Older People) team.

#bgsconf ImageImageImageImage
Funnily enough, having a Comprehensive Geriatric Assessment helped people a great deal!

Well done team! 👏👏👏

#bgsconf Image
The team worked with the Maggie’s centre to develop classes and holistic care for the patients attending the Beatson Oncology Centre.
