My vaginoplasty was a success! 🏳️‍⚧️ Thank you everyone for all the support & kind words.

This'll be an ongoing thread about my experience/recovery, both as a resource to other trans folks, and for anyone who wants to learn a bit about the procedure. Kind/Genuine questions welcome. Image
[Day 0, pre-Op]

I showed up 90 min prior to surgery for paperwork. Then I had my vitals taken, and got my IV. I then met with my surgeons & anesthesiologist.

I did well until 10 min prior to surgery, when I got lonely/scared and started crying a lot.

Anxiety: 8/10
Joy: 7/10
[Day 0, recovery room]

Post surgery I was brought to a room to recover from anesthesia.

Apparently I kept asking 2 things: (1) How'd I do in surgery? And (2) I wasn't mean to anyone, was I?

And I love that that's who I am 😅

Anesthesia: 10/10
Pain: 5/10
Emotions: ???
[Day 0, 3 hours post-Op]

As I was waiting for my room to be ready, pain spiked due to the machine used in surgery having pumped me full of air (this is normal). I was given strong pain meds, then sipped water/juice til my room was ready.

Pain (pre-drugs): 6/10
Pain (post): 1/10
[Day 0, 6 hours post-Op]

I was set up in my own private room, where I'll spend the next several days recovering.

Lots of nurses, taking vitals and getting me all set up. Got access to my luggage, which has my phone charger and switch.

Pain:2/10
Groggy/sleepy: 8/10
Emotionally I feel great. Even w/ the pain, it was like a light switch going off in terms of dysphoria. My body just feels like it was meant to be.

I am a little lonely b/c my friends won't be here til it's time to move from hospital to hotel (next week)

[Day 0, 9 hours post-Op]

Good drugs wore off and pain slowly ramped back up to a 6.5/10.

Just took my next dose, so pain will taper back down in the next 30 min, and the sleepies will kick in.

(Oh also, I'm also on a liquid diet today)
Recovery:
1 week in hospital
2 wks in hotel nearby
6 wks 10lb limit
3 months no strenuous activity

Dilations:
0-6 weeks post-hospital: 4x/day
7-12 weeks: 3x/day
3-12 months: 2x/day
1 year post-op; start at 1x/day, taper to 1x/week as comfortable

My advice:

- research surgeons (procedure, results, videos of them talking about surgery)

- Wait lists can be multiple years. Get on one asap if you're interested.

- tak to other post-op trans gals to get a sense of what it's like

B/c my surgery was robot-assisted, I've got some pain in my chest/shoulder from the gas the robot used. 4/10. Normal. should last 1-2 days.

Other pain is medicated to ~2/10, but it spikes to ~7/10 around my next dose. Catheter is also a strange feeling.

[Day 0, 12 hours post-Op]

Day 0 hospital routine: Vitals checked every 4 hrs, meds every 4-6 hrs.

Pain is around a 2/10 to 4/10 on average, peaks at 7/10). Catheter feels strange 😅

Mostly I'm groggy, messaging friends, and basking in the fact that I finally have my vagina 😭

I have friends/acquaintances dropping off food while I'm in the hotel during weeks 2-3.

Once I'm in the hotel, I'll send out the ask for Missoula folks to drop off meals for the first few weeks I'm back home :)

My crotch is wrapped & my vaginal canal is stuffed with gauze. That plus pain meds means I don't feel a ton at the moment.

But what I noticed right away was the absence of what had been there pre-surgery. And that's led to constant sense of joy/peace.

[Day 0, 15 hours post-Op]

Much of day 0 is a mix of vitals, drugs, & check-ins (catheter, IV drip, etc). Still, I napped for 90 min—my first non-anesthesia sleep since the surgery.

Was nice, but I can only be on my back, which is the one position I don't normally sleep in 😅
[Day 0, 16 hours post-Op]

been on a nerve blocker today (& will be for several weeks), as it reduces pain spikes.

But I'm not on it overnight. And while pain levels are fine, I do have additional sensation—but it's mainly the catheter making me feel like I'm peeing nonstop 😅
[Day 0, 19 hours post-OP]

Got some good sleep again. Woke up to get meds and my blood drawn.

The pain in my chest (standard byproduct of robot-assisted surgery) is the biggest gripe at the moment, and having constantly rotating heat pads has helped a ton with that.
Keep in mind that I just had surgery, so this is what I've been told/researched, not my own experience (yet): My vagina will be highly similar to cis vaginas with some differences:

1) Like cis women, I will have 6-9" of depth in the vaginal canal

2) Cis vaginas tend to have a pH of 4-4.5, whereas mine will be ~7

3) I'll have some natural lubrication, but it will likely not spike much with arousal. I was told it will likely be enough for fingering/oral, but vaginal sex will likely want lube.

4) I'll be dilating my whole life (4x/day to start, down to 1x/wk at 18 months). It is comparable to having vaginismus.

5) Post healing, it should be visually indistinguishable from cis vaginas, outside of some scars between my labia majora/minora.

Obviously this varies person to person, but for me bottom dysphoria was always there. Initially surgery was a longterm goal, but it became more urgent as I started passing full-time—it began to feel like the last step to living fully as me.

[Day 0, 22 hours post-Op]

Gas pain got pretty bad around hour 20-21 (~7 out of 10), but some extra strength Tylenol plus gassex brought that down to 3/10.

Also had my first nerve zap. As I recover, I was told to expect to feel some body zaps around the area as nerves reconnect.

One of the weird battles has been feeling like I need to cough or clear my throat (I was incubated for surgery), but knowing full well that coughing would be painful throughout my body.

So far, no coughs. Though I did hiccup twice, and that was ouchie.
This looks like a p*rn, fet*sh, or s*ssy account, so I'm gonna block and move on.

Sex is like 4 of 5 items down the list as to why I got a vaginoplasty. It's very uncomfortable to be asked about my vagina from someone whose timeline is mostly trans p*rn.

- struggled to sleep the night before surgery (nervous/anxious)
- 90 minutes prior to surgery was full of meetings with nurses, my surgeons, and my anesthesiologist
- 10 minutes before surgery, I got very anxious/emotional, and cried

- I was walked to the Operating Room, which was enormous and felt both sacred and sterile
- Laid down on the table, and was asleep within 2 min
- Woke up a few hours later in the recovery room to come down from anesthesia

- In recovery, I was groggy & in pain (4-7 out of 10). Was given pain meds, then waited a few hours for my room to be prepped

- Once in my room, it was a regular rotation of vitals, meds, and check-ins from nurses & surgical team

- crotch pain ranged from 1-7 depending on if I had meds
- catheters are weird
- despite above, I was euphoric

I knew I wanted a peritoneal vaginoplasty, so I watched interviews w/ surgeons (noting how they talk about it & trans ppl). My surgeon raved about it and I had this sense (bias?) that robot assisted > no robot (consistency in results; reduced scarring)

Graduated to solid food. After meal and pain meds, I will stand up for the first time post-surgery. Image
Upside of running this thread: demistyfing the process for trans women, as well as other folks who are curious about what it entails.

Downside: every 30-60 min, I've gotta block some ch*ser/s*ssy account who sees my hospital selfie and thinks "ah yes, i will f*tishizs this" 🙄
[Day 1 Post Op]

The good pain meds come every 6 hours now (was 4). The 2 hour wait was rough toward the end—solid 7/10 for the last 45 minutes. Feeling better now.

Also, I passed gas for the first time since surgery, which is an important milestone they'd asked me to track 💨

Now that I'm on solid food, I get access to the full menu, and wow it's amazing. So much good stuff 🤤 Image

Following breakfast my surgeon's head nurse came in to walk me through the dilation process, as well as the full care routine once I leave the hospital.

I've still got a few days before dilation/leaving, but she wanted to get that information in now.

She gave me a lot of information, I got to hold the dilators. But most that stuff I will talk about later next week once I'm experiencing it.

Main thing to know was my catheter comes out Monday evening, and I have until Tuesday morning to pee or it goes back in.

After that meeting, another nurse came in I moved from the bed to a fancy chair w/ a butt cushion (both to get used to moving, and also to help with the gas leaving my body).

That sucked a lot. Pain was 8.5/10. Discomfort in chair. Winced until I fell asleep
[Day 1 Post-Op]

Still in chair. Dozed on/off. Pain between 4-7. Primarily gas pain as it's dissipating more today. Some crotch pain as my pain meds wear off.

Also had therapy w/ my regular therapist. Was really good to see her. My body is in pain, but she calmed my heart.

Honestly, the chair has been the hardest part so far. I didn't expect to get so wiped moving less than 5 feet, or for sitting to be so rough (I'm ready for the gas to be gone).

Definitely has caused a bit of a mood shift. I find myself grumpy and wanting sleep.
Laparoscopic surgery uses carbon dioxide to inflate the abdomen to make room for the robot during surgery. This is true of the robot-assisted vaginoplasty, as well as other procedures.

The carbon dioxide takes a few days to move around & exit the body.

I do have insurance. Procedure is costing me $4,250. Then $3,500 for hotel (insurance reimburses $1,500 for travel). Other costs are $750 for post-surgery supplies, $1,500 for flights, & $500 for 1 missed week of work (9 more weeks of PTO covered by FMLA)

[Day 1 Post-Op]

Some recap. If getting to the chair wiped me, getting back to bed was a whole new struggle.

I was exhausted, grumpy, and full of gas pain. Quickly slipped in and out of sleep for a few hours, only to wake up miserable each time
[Day 1 Post-Op]

Over the night I got very nauseous. Threw up twice, and was in/out of sleep. All around bad time.

Plastic surgeon team checked in on me that morning. My stomach is still bloated from gas, they want me on soft foods (I'd been struggling to eat), goal was to poop.
[Day 2 Post-Op]

Lots of sleep in the morning, got fluids and anti-nausea meds. I was very low energy.

Around 11am, nursed helped me use the restroom, and then we took a lap around the ward (my first big walk!)

In the chair again now, and feeling a lil better than yesterday.
[Day 2 Post-Op]

Urological surgeon just checked in; said the surgery went prefect. (Dr. Bluebond does the vaginoplasty/labiaplasty; Dr. Zhao does the urological surgery)

He then talked through next steps of recovery in the hospital, and made me feel like I'm on track :)
Day 0
Gas: 4/10
Crotch: 1-7/10 (oxycodone helped)
Nausea: 0/10

Day 1
Gas: 6-7/10
Crotch: 1-7/10
Nausea: 9/10 (night only)

Day 2:
Gas: 2-3/10 (BM and walking helped)
Crotch: 2-5/10 (off narcotics)
Nausea: 1/10 (on meds)

I'm on a drain, a catheter, and an IV for my stay at the hospital. But after this week, nope, it'll just be me, a variety of meds, and some dilators

[Day 2 Post-Op]

Have done much better this afternoon. 6 hours in the chair compared to 4 yesterday, and far less pain. Used the restroom again, a good sign that things are moving the right direction.

Plan is to eat dinner, then move to bed, hopefully for a good night's rest.
[Day 2 Post-Op]

Night 2 was better. No vomiting, but gas pain still very present.

I don't remember much of the night, as I was in/out of sleeping, but I'm glad it was okay. I was very nervous that night 2 would be as bad as night 1, which sucked
[Day 3 Post-Op]

One thing I haven't touched is just how hard it is having really low energy. Your body is in recovery mode (& rightfully so; surgery requires a lot of healing)

I bring this up b/c even tho I was less nauseous today, I had really low energy. Was moving real slow.

I walked around the ward twice (once with nurse's help, once later without)

Moving is slow, but feels good. And helps dissipate the gas pain, which was really bothering me today. I never thought gas pain would be my main source of discomfort, but here we are.
[Day 3 Post Op]

Although I was feeling better overall, gas pain was really bothersome (4/10 nonstop). It made it hard to eat/drink, as it felt like everything was stuck in my throat.

Exhausted. Ended up cutting therapy short, and going back to bed, hoping night 3 would be okay.
[Day 3 Post-Op]

Night 3 was meeehhhhh. Gas pain still sucked, was in/out of bed trying to use the restroom.

Got up once and had to pause bathroom break b/c I couldn't stop burping, which ultimately turned to throw up (an impressive feat given I was on 2 anti-nausea meds).
[Day 4 Post-Op]

Woke up with gas pain down to a 1/10, though everything is rumbling lots.

Surgical team came and took the wrapping off this morning. Stung a little having all that tape come off. But I feel fine.

Goals for today: shower, walk, and drink lots of juice/water.
[Day 4 Post-Op]

Took a shower for the first time since surgery; was lovely. Also did 2 laps around the ward, unassisted. Ate some solid food, but energy is still pretty low.

Gas pain was mostly gone, but is creeping back in as the evening wears on. Hopefully it'll go away soon.
[Night 4 Post-Op]

Catheter came out last night. Spent the whole night anxiously peeing in the hopes it wouldn't have to go back in.

Sadly, my bladder was only emptying about halfway, which meant the catheter had to go back in. I'll have it in for another week at the hotel.
[Day 5 Post-Op]

Packing removed from vaginal canal. Probably a 5/10 pain with spikes at 8/10.

Honestly, it felt like the surgeon was manually rewinding a cassette tape in my vagina. Absolutely bizarre feeling.

I'm seeing a lot of far-right misinformation about a CPS case in Montana, and I just want to note a few things:

(1) CPS cases are private to protect the child. I won't comment on details of the case, but every indication I've received is that CPS followed proper procedures.
(2) Gender-affirming care is not banned in Montana, as the article (& @GovGianforte) claim.

SB99 was blocked by the courts for being discriminatory. Claims that the child was taken out of state to get gender-affirming care are untrue, as that care could've been received in MT.
(3) @GovGianforte says he's monitoring the case.

While many of us are monitoring the case, it is important to note that this was a court ordered removal, and my understanding is that the Governor does not have a say in the courts determining if a home is safe for a child.

Late last night, the House adjourned Sine Die—meaning the 68th legislative session has come to a close.

As soon as we adjourned, I returned to my seat in the People's House to celebrate the work my colleagues & I did this session, and begin discussing what comes next.
Also thank you to everyone who sent flowers.

While the GOP's undemocratic censure kept me from my seat in the House for the last week of the session, your love & support filled that room—and for that I am so incredibly grateful. Image
