[Day 0, pre-Op]
I showed up 90 min prior to surgery for paperwork. Then I had my vitals taken, and got my IV. I then met with my surgeons & anesthesiologist.
I did well until 10 min prior to surgery, when I got lonely/scared and started crying a lot.
Anxiety: 8/10

Post surgery I was brought to a room to recover from anesthesia.
Apparently I kept asking 2 things: (1) How'd I do in surgery? And (2) I wasn't mean to anyone, was I?
And I love that that's who I am 😅

As I was waiting for my room to be ready, pain spiked due to the machine used in surgery having pumped me full of air (this is normal). I was given strong pain meds, then sipped water/juice til my room was ready.

I was set up in my own private room, where I'll spend the next several days recovering.
Lots of nurses, taking vitals and getting me all set up. Got access to my luggage, which has my phone charger and switch.

Emotionally I feel great. Even w/ the pain, it was like a light switch going off in terms of dysphoria. My body just feels like it was meant to be.

Just took my next dose, so pain will taper back down in the next 30 min, and the sleepies will kick in.
(Oh also, I'm also on a liquid diet today)

My advice:
- research surgeons (procedure, results, videos of them talking about surgery)
- Wait lists can be multiple years. Get on one asap if you're interested.
- tak to other post-op trans gals to get a sense of what it's like

B/c my surgery was robot-assisted, I've got some pain in my chest/shoulder from the gas the robot used. 4/10. Normal. should last 1-2 days.
Other pain is medicated to ~2/10, but it spikes to ~7/10 around my next dose. Catheter is also a strange feeling.

Day 0 hospital routine: Vitals checked every 4 hrs, meds every 4-6 hrs.
Pain is around a 2/10 to 4/10 on average, peaks at 7/10). Catheter feels strange 😅

I have friends/acquaintances dropping off food while I'm in the hotel during weeks 2-3.
Once I'm in the hotel, I'll send out the ask for Missoula folks to drop off meals for the first few weeks I'm back home :)

My crotch is wrapped & my vaginal canal is stuffed with gauze. That plus pain meds means I don't feel a ton at the moment.
But what I noticed right away was the absence of what had been there pre-surgery. And that's led to constant sense of joy/peace.

Much of day 0 is a mix of vitals, drugs, & check-ins (catheter, IV drip, etc). Still, I napped for 90 min—my first non-anesthesia sleep since the surgery.

been on a nerve blocker today (& will be for several weeks), as it reduces pain spikes.
But I'm not on it overnight. And while pain levels are fine, I do have additional sensation—but it's mainly the catheter making me feel like I'm peeing nonstop 😅

Got some good sleep again. Woke up to get meds and my blood drawn.
The pain in my chest (standard byproduct of robot-assisted surgery) is the biggest gripe at the moment, and having constantly rotating heat pads has helped a ton with that.

Keep in mind that I just had surgery, so this is what I've been told/researched, not my own experience (yet): My vagina will be highly similar to cis vaginas with some differences:
1) Like cis women, I will have 6-9" of depth in the vaginal canal

2) Cis vaginas tend to have a pH of 4-4.5, whereas mine will be ~7
3) I'll have some natural lubrication, but it will likely not spike much with arousal. I was told it will likely be enough for fingering/oral, but vaginal sex will likely want lube.

4) I'll be dilating my whole life (4x/day to start, down to 1x/wk at 18 months). It is comparable to having vaginismus.
5) Post healing, it should be visually indistinguishable from cis vaginas, outside of some scars between my labia majora/minora.

Obviously this varies person to person, but for me bottom dysphoria was always there. Initially surgery was a longterm goal, but it became more urgent as I started passing full-time—it began to feel like the last step to living fully as me.

Gas pain got pretty bad around hour 20-21 (~7 out of 10), but some extra strength Tylenol plus gassex brought that down to 3/10.
Also had my first nerve zap. As I recover, I was told to expect to feel some body zaps around the area as nerves reconnect.

One of the weird battles has been feeling like I need to cough or clear my throat (I was incubated for surgery), but knowing full well that coughing would be painful throughout my body.

I knew I wanted a peritoneal vaginoplasty, so I watched interviews w/ surgeons (noting how they talk about it & trans ppl). My surgeon raved about it and I had this sense (bias?) that robot assisted > no robot (consistency in results; reduced scarring)

The good pain meds come every 6 hours now (was 4). The 2 hour wait was rough toward the end—solid 7/10 for the last 45 minutes. Feeling better now.
Also, I passed gas for the first time since surgery, which is an important milestone they'd asked me to track 💨

She gave me a lot of information, I got to hold the dilators. But most that stuff I will talk about later next week once I'm experiencing it.
Main thing to know was my catheter comes out Monday evening, and I have until Tuesday morning to pee or it goes back in.

After that meeting, another nurse came in I moved from the bed to a fancy chair w/ a butt cushion (both to get used to moving, and also to help with the gas leaving my body).

Honestly, the chair has been the hardest part so far. I didn't expect to get so wiped moving less than 5 feet, or for sitting to be so rough (I'm ready for the gas to be gone).
Definitely has caused a bit of a mood shift. I find myself grumpy and wanting sleep.

Laparoscopic surgery uses carbon dioxide to inflate the abdomen to make room for the robot during surgery. This is true of the robot-assisted vaginoplasty, as well as other procedures.

I do have insurance. Procedure is costing me $4,250. Then $3,500 for hotel (insurance reimburses $1,500 for travel). Other costs are $750 for post-surgery supplies, $1,500 for flights, & $500 for 1 missed week of work (9 more weeks of PTO covered by FMLA)

Urological surgeon just checked in; said the surgery went prefect. (Dr. Bluebond does the vaginoplasty/labiaplasty; Dr. Zhao does the urological surgery)

Have done much better this afternoon. 6 hours in the chair compared to 4 yesterday, and far less pain. Used the restroom again, a good sign that things are moving the right direction.

Night 2 was better. No vomiting, but gas pain still very present.
I don't remember much of the night, as I was in/out of sleeping, but I'm glad it was okay. I was very nervous that night 2 would be as bad as night 1, which sucked

One thing I haven't touched is just how hard it is having really low energy. Your body is in recovery mode (& rightfully so; surgery requires a lot of healing)

I walked around the ward twice (once with nurse's help, once later without)
Moving is slow, but feels good. And helps dissipate the gas pain, which was really bothering me today. I never thought gas pain would be my main source of discomfort, but here we are.

Got up once and had to pause bathroom break b/c I couldn't stop burping, which ultimately turned to throw up (an impressive feat given I was on 2 anti-nausea meds).

Woke up with gas pain down to a 1/10, though everything is rumbling lots.
Surgical team came and took the wrapping off this morning. Stung a little having all that tape come off. But I feel fine.

Woke up with gas pain down to a 1/10, though everything is rumbling lots.
Surgical team came and took the wrapping off this morning. Stung a little having all that tape come off. But I feel fine.

[Day 4 Post-Op]
Took a shower for the first time since surgery; was lovely. Also did 2 laps around the ward, unassisted. Ate some solid food, but energy is still pretty low.
Gas pain was mostly gone, but is creeping back in as the evening wears on. Hopefully it'll go away soon.

[Night 4 Post-Op]
Catheter came out last night. Spent the whole night anxiously peeing in the hopes it wouldn't have to go back in.
Sadly, my bladder was only emptying about halfway, which meant the catheter had to go back in. I'll have it in for another week at the hotel.

Packing removed from vaginal canal. Probably a 5/10 pain with spikes at 8/10.
Honestly, it felt like the surgeon was manually rewinding a cassette tape in my vagina. Absolutely bizarre feeling.
