🚨 Concussion Thread 🚨

In light of the #TuaTagovailoa injury let's discuss the most taboo injury in all of sports

❕What is a concussion?
❕Did #Tua suffer from one
❕Were the #Dolphins at fault?

👇
A concussion is a metabolic condition of the brain resulting in cognitive and vestibular deficiencies

Memory loss, visual deficits, balance problems, dizziness, vertigo are common symptoms of a concussion

A headache alone is NOT a concussion
A concussion is typically sustained via trauma to the head; a direct blow to the head or the back of the head hits the ground

A concussion can be sustained without a direct blow to the head via whiplash
As you can see from the video from Sunday, #Tua hits his head on the ground and immediately loses balance and must be held up by a teammate
Per the NFL rules, any visible evidence of a concussion (gross motor instability) they are considered to have a concussion and are removed from the game.
This particular case was ruled to be back spasms.

In my entire career I've yet to see an athlete slam their head on the ground and stumble around because of a back spasm.

In my opinion, I believe this is a complete dereliction of duty by the #Dolphins
#TuaTagovailoa should have never returned to that game much less play on a short week the following Thursday.

Let's fast forward to last night now
Again #Tua hits his head on the ground and begins to posture.

This is textbook decorticate posturing which is symptomatic of moderate to serve brain trauma
Now I do not think that the head injury he suffered Sunday led to the subsequent injury last night. That was a vicious hit

But had he been ruled out Sunday he would not have been on the field last night

I do think #Tua will recover much slower as a result of both of these
Let's not forget that #TuaTagovailoa is still a kid.

He is 24. The male brain finishes development at 25.

Everyone should pray for a quick recovery.
It blows my mind that the #Dolphins staff and an independent neurologist allowed him to return to the game after the first injury.

So yes I believe the #Dolphins are at fault for this and I hope they haven't wasted the career of an incredible young athlete.

Finally wrapping up this modalities series.

We'll hit up on a few misc ones we've yet to get to:

⏺️Voodoo floss
⏺️Marc Pro
⏺️Normatec
⏺️K-tape

👇
Voodoo floss

Claims to decrease the perception of tightness and to reduce soreness via assisting with fascial gliding

Data is very recent so there isn't much to be validated in studies but what is out there suggests this is helpful with improving ROM
Another benefit of voodoo floss is the blood flow restriction (BFR) aspect of it.

There is excellent data showing BFR can help create a harsher cellular environment to stimulate growth and healing.

☀️GM ☀️

Time to continue the modalities train.

We'll be discussing the manual therapies of the modality world today; dry needling, scraping and cupping:
⏺️Dry
⏺️Wet
⏺️Fire

Let's roll

👇 Image
Cupping works via myofascial decompression

This leads to improved circulation and decreased pain

This is a fancy word for saying the tissue is lengthened. The opposite of massage
Dry and fire cupping work the same way; one just makes you look cooler

They just use a different medium to create a vacuum to pull on the tissue

☀️Another day, another thread. ☀️

Modalities continued.

We'll jump into the cool ones today:

Ultrasound and laser.

👇 Image
We'll start with laser as its absolutely the coolest modality there is.

Therapeutic laser uses electromagnetic radiation which the mitochondria of the cells absorb.

This is then dissipated as heat so the therapeutic benefits will be similar.
In short,

Laser shoots light at the cells ▶️ cells absorb light

Cells then convert light to heat

Alright.

🚨Modalities in depth thread starts now. 🚨

Gonna lead off with ice (💩), heat, and electrical stimulation.

👇 Image
First I'll need to address the Gate Control Theory as most therapeutic modalities work via this mechanism.

Essentially it allows the CNS to override a painful stimulus.
Lets say you have Stimulus A (pain).

You then provide Stimulus B (ice, heat, stim, rubbing the injury with your hand)

The new stimulus takes priority and the pain decreases or goes away.

Yes its that simple. Image

🚨 Therapeutic Modalities 🚨

Short thread here to discuss them briefly and let you know whether they're useful or not.

I'll dive into the specifics in the future.

Follow along

👇
🔵Ice - Dumb. Don't use

🔵Hot packs - Might make you feel good and improve circulation to a minor extent. You can do much better

🔵Ultrasound - Really good if you wanna heat a 2x2" area to a very minor extent and waste 7 minutes
🔵Electrical stimulation - Reduces pain for 30-45 minutes. Fine if you only care about pain. Won't fix you

🔵Foam rolling - May help with the sensation of stiffness. If it helps YOU then continue. Not the best

🔵K tape - Psyop by the CrossFitters to look busy

Guess I'll finally finish up the knee thread.

This part will be a bit shorter as I'll just list a few exercises to help restore ROM and proprioception.

See below the link to the rest of the threads on the knee and acute care.

👇

We'll lead off with heel slides.

I like doing these up on a wall to allow some over pressure from gravity.

Fully flex the knee up until the point of pain and then hold there for 2 seconds. Then extend the knee and continue.

Do this for 5 minutes instead of a reps/sets scheme Image
Knee extension is lost less than flexion and I've found I can restore full extension with quad strengthening.

However, if extension is lost a knee extension stretch can help.

This can be done passively which makes it convenient. Image
