For context, DeFormed is a project I hacked on with the amazing team at @mintkudosXYZ for @ETHGlobal Bogotá.
It was an amazing kickoff weekend for my Devcon vacation, and I'm so grateful to the team for inviting me!

We built a prototype in less than 24 hours and demo'd it to a panel of awesome judges.

The flow:

Trying out web3auth was cool because of its flexibility.
You can log in with OAuth via social media accounts, you can use a fresh email, or you can connect with an existing wallet. In any case you get an eth address!

2. Create form
Customize questions and response types with an intuitive UI.
You can token-gate access to your forms to only accept responses from certain communities.
For ex: I can create an app for @AlchemyLearn Ambassadors, available only to #RoadToWeb3 PoK holders.

When users fill out your form, on-chain credentials are automatically highlighted.
