Diagnosis?
Acute infarct - Dense MCA sign

The left middle cerebral artery (MCA) is very dense due to a thrombus - compare with the density of the normal right MCA
A dense artery sign may be seen before any change in the infarcted grey and white matter

Low density is already visible in the infarcted area
The hyperdense MCA sign, also known as Gács sign

focal hyperdensity of the middle cerebral artery (MCA) on non-contrast brain CT

👉 is the direct visualization of thromboembolic material within the lumen.
It is thus the earliest visible sign of MCA infarction as it is seen within 90 minutes after the event
