One thing Twitter COVID pundits do not understand about vax surveillance is that our system works better for very weird events far above baseline rates than modest increases in common events.

Vitt has no baseline. Without heparin, it never occurs. Myocarditis.... @anish_koka
In young men is astonishingly rare and with vax is massively elevated

Surveillance can find these signals more easily than even a 10 or 20% increase in a much more common event like MI or stroke

That's v hard to find
Twitter is full of doctors with little experience analyzing data who essentially tweet threads that are total BS.

Defending our incompetent CDC is mostly tweeting about politics not science.

I have to say, I really think a tiny fraction of people have lost their minds with these stupid CR boxes and this silly dream of ventilating all spaces on earth

Consider a few facts
#1 No one has ever proven that running these boxes helps you (uhh kinda a big opening pt)
🧵🧵
It would be easy to randomize families to have the box at home or not, and measure rates of illness 6 months later.

One could even look at diverse endpoints like allergies and quality of life. And electricity bill

Why not run a study if you believe in it? RCT or shut up
2. Everyone is on a mission to remove all "allergens" and "bad things" in air. It is hard to believe that this box is any better than opening your windows once a week to lets some fresh air in, & sweeping up your apartment wrt real clinical outcomes. (Perhaps surrogates better)

I am going to take a minute to explain why the FDA (specifically Peter Marks' & White House actions) on bivalent boosters was such a colossal screw up

Offit explains in today's @NEJM

After primary vaccination was available, the harms of COVID19 plummeted 🧵
Obviously, since risk is related to age, young people post primary vaccination had some of the lowest risk of bad outcomes from COVID19 imaginable

If you factor in that many also had COVID infection, then the absolute risk to them going forward was astonishingly small
Whenever you want to push a medical product in a population at low risk, you need a high bar for evidence.

If someone is sick and dying it might be OK, in some cases, to take a chance, but that's not the case for a 25 year old man

It is never ok to take chances with his health

While its easy to intuitively agree w authors (everything worse these days), I have no idea what paper means

Whole thing anchored on CD index-uses cites to prior patents declining as a marker of destablization.

But there are many reasons to cite or not cite things 🧵🧵
This is the original paper (ref 12 in new paper)

Is that really what destablizing (dramatic, shifting) invention means-- it really swallows cites?

Some well written reviews (dullsville) do that, and poorly written reviews may do that less.
Then there are these word choices to support the argument

Again, there could be many reasons for changing word choice over decades.

OUR NEW PAPER
onlinelibrary.wiley.com/doi/epdf/10.11…
BK & I noticed that some analyses of mRNA myocarditis break it out by age, sex, dose, & brand
20 yo man d2 moderna = not good;

But, others lump young men & old women together

& It matters; lumping hides the safety signal in young men!
🧵 Image
This was the #1 tactic used by people to claim, falsely, that myocarditis post virus was worse than post vax

That WAS NOT TRUE for men aged 18-22 dose 2 moderna

Even this figure was distorted b/c red bar should have used seroprevalance denom. not test + Image
Now we study the landscape
We find concerning rates of myocarditis occur in men 16-22 Image

It's important to be fair in the assessment of Tony Fauci's performance during the pandemic (🧵)

1. He did lie in April 2020. Of course, the moment he did this, he would be ineffective & untrustworthy to many
slate.com/technology/202…
2. He did not engage in a discussion with @MartinKulldorff and @DrJBhattacharya while making sweeping, unprecedented decisions

One thing no one can argue with is GBD was right about schools, and he refused to listen

statnews.com/2021/12/23/at-…
3. His rhetoric was strongly against school reopenings, and his comments kept them closed
