How? Rock the probe (phased array w/ indicator pointing cranially) just below xiphoid (midline). You will be between aorta and IVC. Try to identify Ao first (more medial) and than fan the probe laterally and you will see IVC.

How? Probe movements 🤚

⚠️For IVC US you will need to know how to rock and do fanning. Rock to find RA! Fan the probe more laterally so you can identify IVC! After that sweep the probe so you can be perpendicular to the UVC.

⚠️We struggle with JVD evaluation

If you look up the data, JV exam is inferior to IVC ultrasound in assessing CVP (part of the volume status exam). IVC US has a higher sensitivity (82%) and lower failure rate (10%). Image
IVC US does better compared with other physical exam findings as well. If you rely on CRACKLES to diagnose CHF, you are missing 85% of patients. In case of leg edema - 60% of patients. Additionally, IVC > 2.0 cm has a better specificity (92%) for CHF than BNP > 400 (85%)! Image
