Someone had asked whether sampling for thyroid function test should be done in fasting only in the previous tweet. It has a short answer and a long answer.

Thyroid hormone levels aren't static throughout the day. There's short term day to day variability and longer term seasonal variability. This rhythm also varies between individuals - some have strong rhythmicity, others don't. So the impact of timing varies between individuals. Image
The within day variability (circadian rhythm) is more pronounced with TSH. See the curve with TSH and the relatively flat line with T4 Image
Now why would fasting or fed state matter at all ?

Postprandial rise in somatostatin can inhibit TSH release and can potentially lead to a reduction in TSH. But the post prandial sample is also given at a later time that fasting sample. So is TSH ⬇ due to food or time or both?
This study (done by my senior colleague) is the only one that tested whether food or time or both mattered. They also looked at 3 different assay methods. Day 1 8 AM and 10 AM samples of TSH (extended fasting) showed reduction of TSH. Day F vs 2 hr PP TSH showed even more ⬇ Image
So both timing and food intake contribute to reduction in TSH. So it is a good idea for labs to mention fasting state and time of sampling.
Does that mean all people should give fasting sample at or before 8 AM ?

Just reduction in TSH is not a deal breaker - but if it results in change in diagnosis, it is. Reclassification (normal vs subclinical hypothyroid ) happens when the baseline TSH is near the cut off.
If the baseline TSH is not near the cut off for normalcy(4 mIU/L) , then reclassification is rare (eg. only 6% in a study Scobbo W V Med J. 2004 Jul-Aug;100(4):138-42). Whereas if it is borderline TSH, reclassification can be more (Mahadevan et al)
This reclassification becomes more important in 1st trimester of pregnancy, as the cost of missing a mild thyroid problem is higher and borne by the baby. For non pregnant adults, especially men, this isn't as critical
So timing and fasting matter more to pregnant women and women in reproductive age who are planning pregnancy. For others, the small reduction in TSH won't matter much.
Summary :

- Ideal way to test thyroid function- fasting, before 8AM.
- Matters for TSH only
- Candidate for 'ideal conditions' testing - pregnant woman, ♀ planning pregnancy
- Inter individual, intraday and longer term variability should also be kept in mind

I have always wondered why patients choose quackery, even when the cost is the same or higher than standard of care. Here are some of the reasons in my experience. A 🧵
1. Lure of cure - because they aren’t bounded by the rules that govern us, quacks can promise the moon and the stars. For a chronic disease, the lure of cure is irresistible.
2. Better communication by the quacks - they talk to the patients, rather than talk at them. They are also exceptionally good in the local language.

3. Distrust of authority / establishment

Saw a man with Hypogonadism today ( means he is not able to produce normal amounts of testosterone). He was in his forties, pretty old to find out you don’t have testosterone. So I asked him why he didn’t consult anyone before.
He said he went to a famous academic institute the first time. He was kept as a demo case - for students. He felt like a human Guinea pig and that experience traumatised him so much he never stepped into a hospital for 15 years !
Yes, we doctors learn from patients - but there’s an etiquette, a bedside manner to be followed when dealing with them. Just because they happen to have a rare disease about which we read recently, doesn’t make them a disease with a human face.

A few months back I met a patient who had come for treatment of Sheehan s syndrome. It was just a refill of her pituitary hormone replacement therapy. Her Tamil sounded different. While superficially resembling a Kerala accent, the word choices and pronunciation were distinct.
I asked her where she’s from. She said “ Srilanka .. I came here in a boat decades ago as a refugee”
This was the first time I had met a refugee. While the wealthier Tamils of Jaffna fled to the west, the dirt poor ones ended up in India. She told how difficult life is for the refugees in the camp. They are a group who have slipped through the geopolitical cracks.

I had seen a young woman , a few months back. At 27, she had been diagnosed with diabetes, a few months after her marriage. Her A1c was 11.3 %. Her happy-go-lucky ways seemed like distant past.
Granted, diabetes isn't as scary as some other illnesses - but she was just finding her rhythm in Chennai, having grown up in a village. The inlaws home had some 'laws' - or conventions, which she had a hard time adjusting to.
Diabetes threw a spanner in her baby plans - as conceiving with such high sugars wasn't advisable.
