Unity isn't going to charge a fee for reinstalls. The spirit of this program is and has always been to charge for the first install and Unity has no desire for the same person charging for ongoing installs.
Unity isn't going to charge for fraudulent installs or "install bombing" - Luckily, many of the same techniques for reinstalls work here, and Unity will continue to make them better.
Unity isn't going to charge for automation/test installs (again, the reinstall checks above get Unity a good ways there, and Unity will make it better)
Unity isn't going to charge for charities etc, never the intent -- this one is one where people would have to give Unity a heads up because Unity will have no way of knowing.
Putting their game in a subscription service, Unity wouldn't anticipate being charged the runtime fee.
