Exchange of LinkedIn messages in which I explain to Cotswold Barristers' chief executive why I had asked my question about Mr Smith's insurance in a public forum, and confirm to her that I will not treat our communications as confidential
Subsequent message from Cotswold Barristers' chief executive confirming that Mr Smith's insurance is on Bar Mutual terms, asking me to treat that information as private and insinuating that it would be a breach of professional duty for me to reveal the information

An entirely predictable, predicted and avoidable event. James's Commanding Officer gave evidence at his inquest that James had been "killed by systemic complacency on the part of the British Army"

The CO of the RSOI BG had been offered a full technical clearance of the ranges by the Commander of the Joint IED TF. The offer was declined, because it would have required the firing points to be kept secure thereafter #TooDifficult

The Range Conducting Officer (RCO) of the range on which James was killed asked for permission for his team to stay out at the ranges to prevent IEDs being planted. Permission was refused #TooDifficult

In 2013, following the North Staffs inquiry, @Jeremy_Hunt announced a review into 14 hospital trusts with excess mortality statistics. It was dubbed the Keogh Mortality Review

The review was headed by then-NHS Medical Director, Bruce Keogh. Panels of inspectors descended on each of the 14 trusts. Each panel included lay representatives (dubbed patient and public engagement (PPE) reps)

I was one of the PPE reps who were part of the panel which inspected Buckinghamshire NHS Trust. I was then invited to be part of the team which presented the results of the overall review to the media & concerned MPs on the day Hunt announced the main findings to Parliament
