"We, the undersigned licensed medical and mental health professionals (INCLUDE YOUR ADVANCED DEGREE IN YOUR LAST NAME WITH NO PUNCTUATION), have a high level of confidence that Donald Trump is showing unmistakable signs of moderate to advanced dementia based on his public behavior and informant reports that show progressive deterioration in memory, thinking, ability to use language, and both gross and fine motor skills."
"1) Decline from baseline:
Overall, he shows a shocking decline in verbal fluency from his previous baseline. He was once highly articulate, with a sophisticated vocabulary, and spoke in polished paragraphs. Now, his vocabulary is impoverished, he often has difficulty finishing a thought, sentence or even a word. Typical of dementia patients he perseverates and overuses superlatives and filler words.
People who worked closely with Trump during his administration are reporting a shocking deterioration in just 4 years."
"2) Memory:
Forgetting names and dates is normal for people who are aging (like Joe Biden). But the Dementia Care Society says "confusing people and generations" is a sign of advanced dementia. Recently, Trump confused Nikki Haley and Nancy Pelosi. 8 times he said he was running against Obama. He didn't look like he was joking. An example of confusing generations: Trump said his father was born in Germany, when that was his grandfather. Michael Wolff wrote Trump not infrequently failed to recognize old friends."
"3) Language:
a) Trump shows formal signs of disordered speech we typically see only in organically impaired patients. Trump is verbalizing an increasing number of  "phonemic paraphasias. Using non-words in place of real words that may include a fragment of the actual word. For example saying "mishuz" instead of missile, or "Chrishus" insead of Christmas. Sometimes he just uses sounds that don't resemble words at all."
"b) Trump evidences ‘semantic aphasia’ where he uses words in the wrong way. For example, "the oranges of the investigation."
"c) Trump evidences "tangential thinking" where he drifts from one unrelated thought to another, and sometimes tries to confabulate them into a story. But the narrative is literally incoherent. With increasing frequency he degenerates into literal incoherence, where no one can tell what he was trying to say."
"4) Motor:
Trump shows evidence of a "wide based gait" commonly found among patients with dementia. He swings his right leg in a semi-circle as if it were a dead weight. He also show deterioration in fine motor coordination, for example having difficulty drinking a bottle of water."
"We feel an ethical obligation to warn the public and urge the media to cover this national emergency."

Dr. John Gartner said: "This is becoming a weekly ritual: A round-up of the latest behaviors evidencing Trump’s apparent dementia. For the eighth time, Trump announced that he was running against Obama. No one believed it when he said he was joking the first seven times, and he keeps saying it, showing just how deeply disoriented he is and how advanced his apparent dementia has become.
Trump is continuing to show more of these phonemic aphasias: “Venezuero” instead of Venezuela. He is also demonstrating sematic aphasias: “steak mountain or steak hill,” instead of “Snake mountain.” Trump is continuing to slur words. What is even more troubling is how Trump sometimes can’t form words at all but just makes sounds. For example, “Saudi Arabia and Russia will…. bluh-ub-bll….”

"Not every outlet persists in ignoring Trump’s alarming rants and verbal pratfalls. Outlets taking Trump’s obviously loopy state of mind seriously deserve recognition. Salon interviewed John Gartner, a psychologist and former professor at Johns Hopkins University Medical School who contributed to the 2017 book “The Dangerous Case of Donald Trump: 27 Psychiatrists and Mental Health Experts Assess a President.”
Gartner was analytic, specific and frank. He pointed to “‘phonemic paraphasias’ — the substitution of non-words for words that sound similar.” He spelled out his observations:
"Some examples of Trump’s non-words: Beneficiaries becomes “benefishes.” Renovations become “renoversh.” Pivotal became “pivobal.” Obama became “obamna.” Missiles became “mishiz.” Christmas became “Crissus.” Bipartisan became “bipars.” …
Trump also engages in what we call “tangential speech.” He just becomes incomprehensible when he engages in free association word salad speech that is all over the place. Again, that’s a sign of real brain damage, not being old, not being slow, not losing a step ... but of severe cognitive deterioration. What I don’t understand is why those clips aren’t replayed over and over in the mainstream media. Isn’t Trump babbling incoherently the most newsworthy part of his rally? You can be sure it would be if it were Biden.'"

“Who is the president?” Is a standard item on the mental status exam. If you said Obama, as Trump did 3 times, you would be institutionalized
This an example of phonetic aphasia. He can’t pronounce whole words properly and his speech sounds like word salad:
