4/ But my guess is that even in the most competitive specialities most doctors might have one or two, to jump the hoop to get into training. They might then publish one more to get a consultant post?

Someone must be able to rank, everyone clinician on @PubMedOnline by their number of papers and plot it as a graph?

Then encourage trainees not to chase publications?
If one paper is enough to get a job, then don't stress trying to get more.
Then people will only publish who want to?
