Some things I’m learning about primary care in Denmark compared to Canada. A thread. 1/n
Mandatory enrolment. Everyone has a family doctor. Patients don’t have to spend time trying to find a FP. The name of the FP is right there on their CPR (Civil Personal Registration) card, Denmarks' version of a social insurance number. 2/n
Patients may choose to find another FP should they wish. 3/n
Access to specialists. Your FP sends a referral and YOU choose a specialist from a list where you see the estimated wait for an appt. No wasted time being referred to one specialist whose list is too long.4/n
Much higher percentage of quick and even same day appointments. MDs more likely to stay on time.5/n
Denmark's system has well-developed electronic health records + several apps that facilitate quick and portable access to the healthcare system from your computer, phone or tablet.6/n
Family practices in Denmark make effective use of allied health professionals in a team approach. Example: midwives providing primary OB/GYN care in the family practice.7/n
There are roughly 3,500 FPs in Denmark. Government figures suggest they need another 1,500. The govt was slow to increase the number of medical school places. Still needed are more FP residency spots and more FP licenses to practice in Copenhagen.8/n
Patients in Denmark are generally satisfied with primary care but have complaints: 1. FP as 'gatekeeper' restricting access to tests and specialists. 2. FPs reluctant to prescribe antibiotics, preferring to 'let nature take its course.'9/n
Other complaints from Danes: 3. Little prescribing of psychoactive meds like antidepressants for young adults. 4. Right or wrong, patients complain that FPs in Denmark are quick to ascribe emotional causes to physical symptoms.10/n

125 breast cancers are induced over the remainder of the patient's lifetime. 2/n
The benefit-to-risk ratio for biennial screening is 48.5 lives saved per life lost.3/n
A thread on H5N1. H5N1 is a subtype of avian influenza that mainly infects birds, but has been identified in other animals and has caused sporadic infections in humans. 1/n
Current or recent outbreaks have been detected in domestic and wild birds and some mammals across Canada, the US, Europe, as well as Central and South America.2/n

A thread on raw vs pasteurized milk. Pasteurized milk is milk from dairy animals heated to 63C for 30 minutes or 71 C for 15 seconds before cooling. It kills bacteria in raw milk that are harmful to humans including Campylobacter, E. coli O157:H7, Salmonella, Listeria etc.1/n
Pasteurizing milk prevents bacterial infections and saves lives. Infection with these and other bacteria can cause food poisoning. The symptoms can be mild but can also cause serious illness. 2/n
Children infected with E. coli O157:H7 can develop hemolytic uremic syndrome which is life threatening. Pregnant women infected with Listeria are at greater risk of miscarriage, as well as illness and death of newborns.3/n
A thread on #BlackPlastic. Black plastic refers to products made of plastic coloured black due to the addition of carbon black. The latter is produced by the partial combustion of petroleum products. It’s to make the plastic stiffer and less likely to degrade. 1/n
Black plastics are found in consumer products including kitchen utensils, kids toys, coffee lids, take-out food containers, office supplies like pens and staplers, computers and cellphone cases, garbage and recycling containers, and small appliances. 2/n
The most hazardous form of black plastic comes from recycled electronic devices like cellphones and computer sleeves…which contain brominated flame retardants or BFRs. 3/n
A few weeks ago I called @KatharineSmart into question for her use of the term “collapse” because I didn’t look closely enough to see the outlines. I now believe we’re a lot closer to collapse than I realized.
1. ER intermittent closures have been normalized. 2. RNs are leaving in droves and hospitals can’t replace them or spend millions patching a roster.
3. Front line MDs and other professionals are exhausted and see no light at the end of the tunnel. 4. Primary care is buried in paperwork and disrespected by the provinces.
