Albertans voting "conservative", a 🧵 on thinking vrs reality:
Most ABs will state they vote conservative for values like: small govt, less bureaucracy, balanced budgets, and govt spending taxpayer money wisely (fiscally conservative)
But in reality our govt is the opposite
1/15
1 - largest govt in AB hx with the most Ministeries and govt bloat
2 - moves in HC to make it more bureaucratic and bloated - 4-5x the admin now, and w admin being govt bureacrats over health experts or HCWs, and with WORSENING HC ACCESS and OUTCOMES
2/15
3 - Profligate financial waste - spending over a MILLIION DOLLARS A DAY on "consultants", wasting millions and millions without any justification or blinking an eye: 2M on covid essay, 70M on tyelnol it didn't get, 100s of M on sole source HC deals for supporters
3/15
- burgeoning unbalanced budget, and bloating of govt: 4 Ministeries and bureacracies to run a HC Ministry that used to only have one Minister (as but one example)
- spending MILLIONS on trips to the US to curry favour, and on travel to discuss seperating from Canada
4/15
There has been NOTHING FISCALLY CONSERVATIVE about this govt (2 MLAs have even left caucus/cabinet and expressed this very concern already!)

And on top of this, the govt wants to insert itself into patient and parental decisions...
5/15
It wants to interfere with ABs rights regarding health and education:
- inserting itself in the pt-doctor relationship on adddicitions care, and transgender care (forcing care on some, and denying care to others actively seeking it!)
6/16
- restricting access to evidence based options while encouraging the ability for a handful of docs to ignore evidence and do what they want (actively catering to the fringe to allow them to push meds that have no proof whatsoever, i.e. Ivermectin for COVID and Cancer!)
7/15
- while actively MUZZLING and censoring Public Health Leadership in the province. Their policy: Health experts do NOT know what is the best medical advice, untrained and non-expert elected officials somehow do??
8/15
And now they also want to tell parents what books their children can or cannot access. No attempt to work with the school boards on appropriate access controls, or let parents decide what is appropriate for their kids... govt KNOWS BEST.
9/15
The current reality: give AB govt your money and it will grow and bloat and tell you what is best for you and your loved one's health and education. Govt knows best. Big Brother has your best interests at heart, just trust and don't question... cuz we're "conservative"
10/15
I believe in small efficient govt, not wasting our taxpayer money, and being transparent w how the money is spent to ensure there is a robust health and education system, along with a social safety net for those less fortunate...
11/15
I also believe in personal freedoms and choice when it comes to making decisions as to what is best for me and my family, while balancing that with the social contract of giving up some rights to ensure the safety and security of our community...
12/15
I believe experts hold value in society, and that they can help us all when it comes to making the best decisions for ourselves and our family. I don't believe that non-expert elected MLAs can take over the role of expert consensus and opinion as is currently occurring...
13/15
Sadly in AB, our govt's actions are the exact opposite of these priniciples that most ABs hold...
In practice our govt is the farthest thing from fiscally conservative and or libertarian (when it comes to avoiding govt overreach)...
14/15
It is beyond time that ABs stop letting our govt pretend to be conservative, and to take a hard look at what they are doing to our province.
I worry when they are done there will be little left of PUBLIC HC and Education - the very rich will benefit, the rest will suffer.

Albertans should be paying attention and be VERY concerned:
Govt is marching forward with privatizing all of our HC system.
Many many concerns w selling off hospitals to private for-profits:
🧵 1/12
buff.ly/NHDveGp
1 - lack of transparency and accountability: these "deals" get done in back rooms and public doesn't see any details - costs, no public reporting of outcomes and access metrics, no accountability... nada. (look at CSFs - they do NOT have to report like hospitals)
2/12
2 - Lack of Integration/Coordination - there's no operational control for all of these for-profits. Govt will say that it will hold these private owners to account and will make them maintain same standards as the public hospitals, but this already doesn't happen

Real Human EDOC Cases in AB continued, Case 6🧵 :
Elderly Calgarian brought in by EMS w cough + severe abdominal pain,
- offload to ED hallway for hours with pneumonia
- CT abdomen pending because of unexplained abdo pain - takes hours to get done as other cases go first
1/5 Image
- Slowly getting worse while alone in the hallway
- Sadly, the pt decompensates further and abdomen becomes rigid with severe pain
- Very difficult to examine and give pain meds in the hallway. Impossible to properly montior.
2/5
- CT scan ultimately shows ischemic bowel - a very painful and very time sensitive condition
- Ultimately the team manages to find a curtained room to administer end of life care in a chaotic and noisy and busy ED

Should be very interesting... it only has a chance to potentially work if: there is transparent reporting of objective pt-oriented outcome measures.

Right now ABs are literally dying waiting in hospitals for surgery, while govt priotizes joint sx.

This will be on Dr. Eagle and the ACA org to make this work.
Time to ensure prioritizing surgical care in some organized way. Pt's dying of cancer while the quick and easy bone and eye cases are getting done

The system can be agnostic of funding scheme if govt doesn't meddle
2/8
Set priorities, provide appropriate funding levels, and then share transparent outcome metrics to ensure accountability.

Instead govt lurches from back room deal to back room deal, disintegrates the system, and adds artificial and arbitrary "competition"

Congratulations Alberta Government! You have OFFICIALLY KILLED AHS. RIP

"response levels to seven questions about patient safety culture also all showed a unanimous decline in satisfaction"

A JUST and SAFE culture no longer exists.

Albertans - you should be VERY AFRAID
Govt killed our flawed but functional health authority with NOTHING in place to replace it

Covenant Health is even more of a mess (see data), but for ideological reasons is not on govt's KILL LIST... but it cannot fill the void created
2/11
Dr. Eagle is new CEO of ACA - an org he believes will be at the "mezzanine level" and will take months to years to be able to help our failing hospital "service provides" - if the new ACA org can help at all?

So there is NOTHING to replace the slain beast that was AHS.

A couple of key points here🧵 :
- This is a DIRECT PLEA FOR HELP - govt is not listening to the profession. We need Albertans to say "enough" and demand a plan from our govt.
- This can't be dismissed as "seasonal volumes" - this is the unsafe norm
1/7
- govt has been ignoring these conditions in the adult world, but they repeatedly profess they "care about the children"... will this spur them to act?
- EVERYDAY is unsafe for so many pts in our EDs... and it is getting demonstably WORSE over the last 2-3yrs
2/7
- This is NOT 1 or 2 "docs who just don't like change - It's our ENTIRE Edmonton Pediatric Emergency Medicine Specialty group
- This sentiment is shared and is supported by the VAST majority of our ENTIRE Pediatric specialty group.
- Our Adult specialities share it as well

Real Human EDOC Cases in AB continued, Case 5🧵 :
- let's talk about "TWA" = transfer waiting admission = very sick pt in the ED and STRANDED for days waiting to become an EIP (Emerg in-pt = OFFICIALLY stranded in the ED)
1/14 Image
- TWA and EIP - we have offical acronyms to dehumanize and normalize absolutely deviant care of HUMAN BEINGS
- these are ABs in their time of need, our loved ones, who get reported to govt as aggregated stats with the suffering hidden behind acronyms that confuse govt
2/14
Real AB ED Case #5:
- pt in their 80's with falls and too weak to ambulate independently. Has multiple acute on chronic medical issues. Requires admission and specialty hospital care.
- stranded in a Covenant Health ED: designated as a "TWA"
