WE DID IT! This was HARD WORK. Leading this project was HARD. I am immensely grateful for the support of my team, their time and expertise, their patience. Y'all, every doctor in the world needs to read this manual. Let me tell you why in this thread 1/
2/ Infection-Associated Chronic Illnesses (IACIs) like Long COVID, MECFS, and Post-Treatment Lyme Disease Syndrome aren’t “mystery illnesses."

They have biological drivers we can identify, study, and treat. That’s what this manual breaks down for clinicians.
3/ Driver 1: Persistence of pathogens in tissue Sometimes, a virus or bacteria or even parts of it, stick around in the body long after the initial infection, hiding in tissues where the immune system can’t easily clear them. Colorful illustrated scene titled “Pathogen Persistence.” In the center, a text box explains that pathogens can hide in body tissues, avoiding detection in fluids like blood, and that these persistent “reservoirs” may disrupt immune response, metabolism, and gene function. The background shows a whimsical, alien-like landscape with pink and blue hills, strange trees, and a winding path. A cartoon white blood cell wearing a monocle and top hat walks along the path, holding a magnifying glass, while pink cartoon microbes peek out from tissue structures.
4/ Driver 2: Pathogen reactivation under immune dysregulation
Viruses like herpesviruses (which most of us have) or even bacteria can “wake up” when the immune system is off-balance. This reactivation can worsen symptoms or trigger new ones. Colorful cartoon illustration of a long, pink dragon emerging from volcanic mountains, breathing fire toward small, anthropomorphic cells that appear frightened and are running away. The dragon’s body snakes through the mountains and dips into a blue ocean-like area, where part of its body is submerged and its head appears calm and sleeping. The scene represents pathogen reactivation, with the dormant dragon symbolizing a pathogen lying inactive before reawakening to attack.
5/ Driver 3: Mitochondrial dysfunction
Mitochondria are your cells’ energy factories (dare I say, the powerhouse of the cell). In IACIs, they can get damaged or dysregulated, leaving people with crushing fatigue, post-exertional consequences, and muscle weakness. A sad mitochondria with a low battery
6/ Driver 4: Coagulation & vascular dysfunction
IACIs can cause sticky blood clots called microclots and damage to blood vessels. This limits oxygen delivery to tissues and can impact every organ system. Red blood cells with sticky fibrin deposits on them
7/ Driver 5: Dysautonomia & POTS
Your autonomic nervous system controls heart rate, blood pressure, digestion, all the “automatic” stuff. In IACIs, it can malfunction, leading to dizziness, heart palpitations, and exercise intolerance.
8/ Driver 6: Neuroinflammation & cognitive dysfunction
Inflammation in the brain and nervous system can cause brain fog, memory problems, headaches, sensory sensitivity, and mood changes.
9/ Driver 7: Immune activation, dysfunction, & autoimmunity
The immune system can get stuck in “on” mode or start attacking the body’s own tissues. This can perpetuate inflammation and damage. Cartoon illustration labeled “Autoimmunity” with the Mount Sinai Cohen Center for Recovery from Complex Chronic Illness logo at the top. In the foreground, an angry cartoon white blood cell with a monocle and mustache points accusingly at a pink humanoid cell, holding a magnifying glass in one hand. Behind the white blood cell, another immune cell holds a Y-shaped antibody and also appears confrontational. The pink cell being accused looks distressed and is stepping back defensively. Other anthropomorphic cells in the background watch the scene. A “Wanted” poster on the wall shows a spiky p...
10/ Driver 8: Microbiome imbalance & SIBO
The gut microbiome influences immunity and inflammation. Disruption after infection, plus small intestinal bacterial overgrowth, can drive symptoms across the body. Cartoon illustration labeled “Microbiome Imbalance” with the Mount Sinai Cohen Center for Recovery from Complex Chronic Illness logo at the top. The scene features a large, uneven balance scale in a surreal, colorful landscape. One side of the scale is weighed down and dripping with pink liquid and clusters of microbes, while the other side holds fewer microbes. Below the scale, a sad, anthropomorphic bacterium sits on the ground, holding its knee. The environment is filled with whimsical plants, fungi, and flowing pink liquid, symbolizing an unhealthy shift in the body’s microbial community.
11/ Driver 9: Hormonal imbalance
IACIs can throw off endocrine systems, including thyroid, adrenal, and reproductive hormones, which can worsen fatigue, pain, and mood symptoms.
12/ Driver 10: Mast cell activation & immune cell priming
Mast cells can become overactive, releasing histamine and other chemicals that trigger inflammation, allergic-type symptoms, and flares. "Colorful cartoon illustration representing mast cell activation syndrome (MCAS). In a pink and blue landscape, anthropomorphic purple and pink mast cells with ponytails display emotions: one looks angry and points, another appears worried, and a third is sneezing into a tissue. Yellow starbursts and jagged lines float around them, symbolizing the release of histamine and inflammatory chemicals during an allergic-type reaction. A small round figure in the center looks distressed."
13/ This manual walks clinicians through these drivers: the science, the evidence, and how to translate it into patient care.

It’s free, evidence-based, and updated every 6 months. Download now at .iacimanual.org
14/ To my team, collaborators, and patients whose lived experience guided this work: thank you. After living through severe Long COVID, it is an honor to lead this project. Every doctor in the world needs to read this manual, because the science is here, and patients can’t wait.

Turns out if you casually eat about 4 cups of cherries and then go for a brisk walk around the block, you probably won’t make that very important and critical home stretch to the bathroom without projectile pooping on every fiber of fabric covering your body
A true jeopardy moment: do I completely ruin this outfit RIGHT NOW and strip butt naked outside at the front door and throw the clothes directly into the trash, or try to thug it out and risk a new paint job on the house and potential new carpet. ALL BECAUSE OF THE DAMN CHERRIES
Mind you, I WAS NOT AT HOME. I WAS AT SOMEBODY ELSE’S HOUSE!!! Literally the worst LMAOOOOO

I read up on some successful uses for NAD+ in alleviating Long COVID fatigue, and I went ahead and tried it, and WOW. I've been on it for the past month and that thing has steadily been knocking out that fatigue. To the point where I have been working out AND doing a day's work.
This is what I've been taking amzn.to/4bTjDCF
Today I've done more in one day than I have done in over two years and I honestly didn't know I had it in me!! Like, at this rate I can say I'm actually REALLY feeling good on a day to day basis. Also I think my psych meds are finally stabilizing and I'm literally on cloud 9.

Y’all can read about this absolutely distressing situation of me trying to buy a home + learning that the seller didn’t want to sell to me because I’m Black, in @nytimes. Thank you @debra_kamin for your hard work investigating this + educating people about the Fair Housing Act 1/

You can read the article here. There are resources and really important things to know about the home buying process included in the article. I highly recommend you read this.

My real estate agent's BROKER just called me at 9pm on a Friday to tell me that the seller doesn't want to sell a house to me because I am BLACK. This is after the inspection, contract agreement, down payment. Baby, I'm either buying your house or buying YOUR BLOCK. CHOOSE ONE.
We all confirmed, super illegal. My agent and the broker called and they both said in 25+ years of doing real estate they NEVER had this happen. Both are older white dudes, both pissed + sick to their stomach. My agent said he wasn't going to get sleep.
I'm up because I'm honestly livid, irate, irritated. Not the news I was expecting to receive. Image
