With #ASH20 around the corner, I wanted to share a summary of the major myeloma immunotherapy - CARs, Bispecifics, ADCs trials and #ASH20 abstracts to look out for #mmsm from a recent talk I gave bmj.com/content/370/bm…
I will focus on some of the major drugs in this thread (in the blue boxes). Although I may not be able to cover all of them, the trials I have typed over the image are the ones to watch out for at #ASH20
We will see updates at #ASH20 for cilta-cel (JNJ-4528) and P-BCMA 101. We will see subgroup analyses and correlative data for KarMMa and EVOLVE studies.

WHAT IS THE RISK CLASSIFICATION OF THE t(11;14) in MM?
This has been a long-debated question and probably asked in the wrong way. I have always had a special interest in the t(11;14) and want to tell a story and how it relates to venetoclax.

In 1998 we reported the t(11;14) was a poor prognostic marker (only translocation visible in karyotypes). We were seeing the effect of abnormal karyotypes- more growth and tumor burden. @VincentRK

That year I convinced M Gertz to send me to Leiden, Netherlands to study t(11;14) via fiber-FISH- most elegant FISH assay. Only achievable by the great Dutch masters. At my return M Kuehl said “stop wasting your time & start doing interphase FISH.” Best advice!

@mwschoen Dr. Schoen walks us through a hypothesis that renal impairment and racial differences in estimating renal function may impact treatment and outcome: 1) Pts with renal dies less likely to receive lenalidomide. Image
@mwschoen 2) Black veterans have higher creatinine but better GFR... if clinician only looks at creatinine, may err in understanding pts renal function. Image

Abs 2209, Poster today: 1/3 of pts with "good" PS were frail; Frail pts with AML had 50% 100-day mortality! ash.confex.com/ash/2019/webpr…
Abs 1563, poster today: In SEER-Medicare analysis of 2800 older adults with Hodgkin Lymphoma (1999-2014), 1/4 received NO treatment.

Abs 268: @HiraSMian presents data on >5700 pts with MM from Canadian registry with some good/some sobering news for #geriheme pts:
1. ASCT use rising
2. 60% of older pts get a novel drug
3. 20% 1-yr mortality
4. 30% of older patients received NO MYELOMA TREATMENT

Even with universal coverage, older pts, lower SES or more comorbidities less likely to be treated for myeloma #mmsm #geriheme #ASH19 ImageImage
High 1-yr mortality for older pts with myeloma who don’t receive treatment. We saw similar rate untreated in SEER- Medicare database. #geriheme ncbi.nlm.nih.gov/m/pubmed/29429… Image

Abs 69: Vulnerable Elders Survey-13 in cohort of >2000 patients in the Lymphoma Epidemiology Outcome cohort.

28% of pts were vulnerable by VES13. Pts more likely to be vulnerable if they completed VES13 after treatment

@MaurerStats Vulnerable pts had poorer OS (HR ~5) *** independent of performance status***. PS is not enough! #ASH19 #geriheme
@MaurerStats Moderator question: How to use this info? Answer: good tool to screen for need for geriatric assessment

Abstract 67: all new patients with blood cancer age >=75 screened for frailty; participants randomized to receive geriatric consultation. Outcomes: feasibility, 1-yr OS, acute care utilization, goals of care discussion. ash.confex.com/ash/2019/webpr…
86% participated in RCT, but some patients worried that geriatric consultation would make them ineligible for treatment. Just heard this same observation from another #geriheme MD on the opposite side of the country just last night! @ROlinMD
@ROlinMD Outcomes: OS not improved (Abel says geriatrics is not magic!) but geriatric consultation did increase documented goals-of-care conversations. #geriheme #gerionc #ASH19

#HematologyTweetstory 10: how did the 5 major #nucleobases get their names? This is slightly tangential to hematology, admittedly, but is a cool story that I’ve been aching to tell since I went through an intense etymology phase many years ago (still have lots of dusty books)./1
#Guanine is probably the most interesting of the 5. It was first isolated in 1846 from guano, i.e. bird crap, by German chemist Julius Bodo Unger (1819-1885), who studied with the famous Justus Liebeg in Gießen. Unger later owned a factory in #Hannover. Guano image from JSTOR./2
It is hard to imagine just how important seabird guano was economically, back before the beginning of the 20th century when Fritz Haber (who favored #PelgerHuet style eyeglasses😉) figured out how to fix nitrogen artificially & make ammonia. Guano was used for fuel, fertilizer./3
