As of yesterday, I finally wrapped up analyzing the 2022 individual & small group health insurance market premium rate filings for all 50 states + DC.

As of this writing, I have the average *preliminary* rate changes for every state, & the *approved* changes for 19 of them. 2/
Once the remaining 32 states are locked in, the national weighted average rate increase will likely be a point or two lower, but as of right now it's around +3.9%.

This ranges from ⬇️4.8% in Arizona to ⬆️ 15.5% in New Mexico. Individual carriers vary greatly as usual. 3/

#ACA SEP enrollment via the 36 HC.gov states is 2.8x higher than for the same period last year (pre-#AmRescuePlan) and 3.7x higher than in 2019 (pre-COVID).
Middle-class enrollees (>400% FPL income) jumped from 2% of #ACA exchange enrollees to 7% thanks to the #AmRescuePlan's subsidy expansion.

Existing #ACA exchange enrollees are saving an additional $67/month apiece thanks to the #AmRescuePlan!

Sounds like it boils down to:
1. Making ARP subsidies permanent
2. Dental/Vision/Hearing for Medicare
3. Warnock/Ossoff/Baldwin Gap bill?

Wildcard: @SenatorSheehan's #S499 #SilverToGold, which could SAVE money!
The @UrbanInstitute recently estimated #S499 as costing around $350 billion net over a decade...but that *included* making the #AmRescuePlan's subsidies permanent, which are apparently already baked into the #AmFamiliesPlan regardless at an estimated cost of $163B. 1/
The rest of #SilverToGold would presumably cost ~$187 billion...but a more limited version might not end up costing anything net (instead of going to 85/90/95 AV, the upgrade might go to 90/95 only, with everyone over 300% FPL still getting a Gold 80 AV plan, for instance).

Thanks to the #AmRescuePlan, #ACA enrollees earning less than 150% are eligible for $0-premium "CSR 94 Silver" plans in most states...which have extremely low deductibles/co-pays to the point that they're effectively the same as Platinum plans! 1/
ALSO thanks to the #AmRescuePlan, if you're receiving unemployment benefits OR IF YOU DID EARLIER THIS YEAR, you're ALSO now likely eligible for a $0-Premium #SecretPlatinum plan as well!

Some states went live with this already; HealthCare.Gov goes live July 1st!

Good lord. The first 2 anti-expansion speakers during public comment are just spewing utter garbage.
The first Zoom speaker is claiming that if they expand Medicaid, it will somehow cause people to WANT to earn less so that they'll qualify for it. Which might make some sense if the rest of the ACA didn't exist for those who earn more, or if they could afford ANY coverage NOW.
The second Zoom speaker claims that Wyoming has crappy oversight over Medicaid waste/fraud/abuse, therefore they shouldn't expand it. Here's an idea: Expand it *and* improve your oversight process? Just spitballing here.

1. CMS is rejecting a Trump rule re. how maximum out of pocket (MOOP) expenses are increased; by reverting back to the prior formula, the MOOP increase will be $400 - $800 less than it otherwise would have been.
2. CMS has confirmed that those taking advantage of the #AmRescuePlan's COBRA subsidies thru September WILL be able to switch to an #ACA plan for October - December via a Special Enrollment Period! (this is a big deal for several million people).

EXAMPLE 1: A single 26-yr old living by themselves will save up to $123/mo on avg...and if they earn less than $19K/yr, they're eligible for a FREE #SecretPlatinum plan (technically it's Silver, but w/generous cost-sharing subsidies making it the equivalent of Platinum).
EXAMPLE 2: A single 40-yr old will again save up to $1,480/yr on avg., and again, can get a #SecretPlatinum plan either for FREE (if they earn up to $19K/yr) or for as little as $43/mo if they earn up to $25.5k/yr:

I've deleted a hastily-posted, kneejerk response tweet which I shouldn't have posted. I'm just pissed as hell at GOP legislators in states like WY & MO who either rejected Medicaid expansion or are flat-out overriding their own voters on it (as well as FL & TX, of course).
Just yesterday, a 5-member Wyoming Senate committee rejected expanding Medicaid to 25,000 of their lowest-income residents *again* in a 3-2 vote, even though it would cost the state NOTHING for the first 3 years or so & only 10% of the cost after that. acasignups.net/21/03/31/3-wyo…
And this came just a week after Missouri Republicans defied their own voters & state constitution by blocking *funding* of their share of Medicaid expansion *even though they won't have to pay a dime for up to the first 4-5 years*, jeopardizing the program

This is a long, wonky post, but it also includes the simplest explanation I could give of how #SilverLoading works and why a lot of people were eligible for $0 premium Bronze or *Gold* plans even before the #AmRescuePlan expanded them further.

In short: Here's some sample 2017 Bronze, Silver & Gold premium & enrollment data. Let's say 2018 medical trend went up 5%. Meanwhile, let's say 1/2 the enrollees received CSR help averaging $2,400 apiece.

That's $120M in CSR reimbursements owed to the carriers from the feds. Image
In late 2017, Donald Trump cuts off CSR reimbursements, figuring this will cause millions of low-income enrollees to lose their subsidies *or* for all the carriers to drop out of the ACA market (causing it to "blow up" and "collapse"), or both, to avoid eating a $120M loss.

📣 NEW: Time to check in on how the #COVID19 Special Enrollment Period is going...

As @selenasd just noted, and @KrutikaAmin reiterated, the only *official* COVID SEP enrollment tally from HCgov is 206K as of 2/28...plus scattershot data from a handful of state-based ACA exchanges.

My estimates suggest the national total was around 260K as of 2/28...
...or an average of ~18.5K per day. *IF* that pace has held true since 2/28, the national tally would be around 684,000 as of yesterday.

HOWEVER...since 2/28, the #AmRescuePlan passed the House, the Senate, the House again & was signed by President Biden on 3/12!

(NOTE: Earlier I questioned where @MehdirHasan got his $35B figure. That's been answered reasonably, but it also creates a *different* mystery: @USCBO says $22.8B. @JCTGov says $35.0B. Which is right, and why would they differ when they collaborated on the same score??)
Having said that, even w/JCT est. of $35B, @MehdirHasan still claimed it'd only cover "2M people for 6 mo" which would be $17.5K/6 mo or $35K/yr apiece.

Problem? The House version estimated *3M*, not 2, and the *Senate* version would cover FAR MORE people (thus the higher cost).

.@mehdirhasan is a great interviewer and is often dead-on target, but he (like so many others in the news media) MASSIVELY overstated the cost per enrollee of the #AmRescuePlan's COBRA subsidies last night (I'm still working on the post but this is key):

Hasan claimed that the #ARP will spend $35B to cover "around 2 million" COBRA enrollees for 6 months.

$35B / 2.0M = $17,500 apiece. For 6 months.

If true, this would indeed be a massive waste of money, since employer policy premiums average ~$7,500 per *YEAR* per enrollee.
On the upside, claiming COBRA costs $35K/year is a lot better than claiming it costs $80K/year, I suppose. He only overstated the cost by 5.6x instead of 10.6, so I guess that's progress...

Hoo boy. Again, I’m not a fan of COBRA but there are legitimate reasons for doing it this way in this particular situation.
First, I'm not sure where @MehdirHasan gets $35 billion from. According to the @USCBO score of the final Senate version of the #AmRescuePlan, the gross cost of the #COBRA provision is $37.2 billion (slightly *higher*), but the *net* cost is $22.8 billion:

Second, the *net amount being spent per COBRA enrollee*, according to the CBO, was around $2,600 apiece under the *House* version *$7.84B / 3.0M) to cover 85% of their premiums. If you ignore savings (why?), the *gross* amount was around $4,800 apiece:

#HR1790 would reverse the Trump Admin's decision to modify how ACA subsidies & maximum out of pocket expenses are calculated. The Trump version made subsidies weaker/costs higher; reversing it would make subsides stronger/costs lower.

Crossing my fingers and hoping that @SenatorShaheen's #S499 is included in the package. Not only would it make the #ARP's subsidy expansion permanent, it would also upgrade #ACA benchmark policies from Silver to Gold...and it would mostly pay for itself!
