#COVID19 tweet after a long time.
Generally COVID is by the by now
More use of mask than before
Lot of public/closed spaces activities
What is worrying for #Bengaluru is this-->
deccanherald.com/city/top-benga…
Going back to pvt hospitals to release more beds is talked about. Non COVID conditions needing ICU will suffer equally. Increased mobility and activity will see more road accidents, other infections..
Difficult balance between livelihood and lives, and then lives and lives too!
We seem to be functioning in a 'let's see what comes our way today' format, moved away from planning and strategising. Will this prove costly? #COVIDBangalore #COVID19karnataka @epigiri

An open letter to @PMOIndia @narendramodi

As the number of cases rage to 700k+, with all our major cities under the firm grip of virus, it is imperative that we consider and take all emergency measures in our arsenal!

We have one very important tool completely ignored.
#Ivermectin a FDA approved "Wonder Drug", already in use for multiple Mass Drug Administration Programs worldwide & even in India, has shown amazing potential in reducing the viral load of #COVIDー19

Multiple clinical trials are already underway, and all of them show +ve result
A comparative study with #HCQ in South Florida, showed ~40% reduced mortality when patients were treated with #Ivermectin!

Another in Bangladesh shows 100% recovery rate on patients treated with #ivermectin Ivermectin.

Every person diagnosed to have #COVID19 should have a kit comprising of pulse oximetre , thermometer, medicines, a list of do’s and don’ts. The state Governments can procure and supply to every person diagnosed at their home ahead of home isolation. @CMofKarnataka @drashwathcn
The state Government has notified list of hospitals for #COVID19karnataka admissions & nodal officers in charge for any complaints. No person can be denied beds. 2 of N #Bengaluru
The BBMP has to display the actual beds in each hospital in real time basis. It is impossible to believe that they couldn’t get an app in time despite #Bengaluru called as Silicon Valley. Only then, private hospitals that are not admitting patients can be held accountable. 3 of N

#HomeIsolation #Covid19Karnataka on cards is a welcome move
It precludes >50years and those with comorbidities = fair enough
It precludes #ChildrenwithCOVID <10 years of age = Needs Discussion!
Impractical to admit (Asymptomatic/mildly symptomatic) children<10yrs for 10 days on their own
Worldwide data = hardly any morbidity/mortality in this age gp
Data = suggests they are not super spreaders
Both clinically and epidemiologically Kids can stay home.
@epigiri @DHFWKA
#HomeIsolation age criteria needs a rethink @DHFWKA @Suraksha_Pinnu @ranjanikanth_ If as paediatricians we are not advocates of children's emotional and physical well being - we would fail them. @iapindia

Anyone knows who is in the expert Commitee to define community transmission? Image
How much of use is X ray to screen covid19 while cases are surging? The @BSTImaging Radiology decision tool for suspected COVID-19 indicates its for persons with lower respiratory tract involvement. Why is the Government proposing this for screening in Karnataka?
2ofN Image
Just when cases are surging, multiple command lines are confusing officials and implementing staff. Unity in command is a must when state has to take decisive actions. @CMofKarnataka @BSYBJP should lead the efforts with support of group of ministers.

MANAGEMENT PROTOCOL
FOR HOSPITALIZED PATIENTS

Prepared by
Dr. Pradeep Rangappa
Dr. Karthik Rao
Dr. Prathiba G.A
ICU TEAM

Valuable Inputs by
Dr. Sunil Karanth
Dr. Chandrashekar T R ImageImageImageImage
Using #TeleICU guided by the above protocol (earlier version), @RangappaPradeep and team guided half of Karnataka districts to have much enviously low CF of 0.54, less than the state average CF of 1.54.
#Covid19Karnataka

#Bengaluru Has the third largest CF in the country and worsening the indicators for the Karnataka state. When the state has implemented the #TeleICUs and reduced deaths in other districts, why not Bengaluru city is not adapting it?

1/n Few Suggestions #Covid19Karnataka #BedManagement
@DHFWKA @BBMPCOMM must have everything set. Yet,
1. Triage Task force - risk stratification - assign home/centre/hospital for initial care. Patients enter triage in two streams: New and already known(Home etc).
2/n
2. Bed management task force: Single decision maker, but have reps of all med colleges and large pvt hosp - avoids refusal from any such centre
3/n
3. Transport committee: 108, police, pvt ambulances and non ambulance dedicated vehicles. GPS enabled, parked at 8-10 points in the city. Activated as soon bed is located and blocked.

While the cases are surging, it’s in our control to keep Namma Bengaluru as best model despite the vast population of the city.

1. Wear masks, it can mitigate burden on health system in terms of how many severe cases it can handle.

2. If you have symptoms of breathing difficulty, fever, cough or cold, use Apthamitra app of Karnataka Government or Call 14410 for medical help. Download here: tinyurl.com/aptmitra from App Store or tinyurl.com/apthmitra

3. Remember, more than 80% of persons with #Covid_19 do not need hospitalization or immediate active intervention. Do not overcrowd the hospitals. As responsible citizens, contribute to keep the beds available for only those need it.
