1) Explain this: The life-saving leukemia drug imatinib costs only $480 per year at @costplusdrugs @mcuban

Instead of the $100,000+ retail price. How? Why?

3) Imatinib is a life saving drug for chronic myeloid leukemia. The poster child for successful targeted cancer treatment. It was introduced in 2001 at $32000 per year.

Price rose to >$100,000 by 2012. Lesson 1: Price of old drugs in USA increase markedly for no apparent reason

Lenalidomide cost per month

$100 in India
$1000 in Europe
$18,000 in USA

No wonder lenalidomide is the #2 drug Medicare spends the most money on. #CostVR @DavidP4AD @policydeb
1/
Why?

17-20 generics in Europe and India bringing down the cost.

2-3 in USA but they are prevented from getting full market share.

I led the first published phase II trial of Lenalidomide. So my interest in the cost is more than cursory. ashpublications.org/blood/article/…

#4 CAR-T targeting GPRC5D. Doubt if one BCMA approach fails another BCMA approach will give significant benefit. These treatments are incredibly expensive. We need immunotherapy options that target something besides BCMA. @ZJU_China

What do you pay for your meds in the US? I checked commonly used med prices on @GoodRx. Prices vary a lot!

@Lisa_L_Gill did this analysis a few years ago.

I added @costplusdrugs @mcuban to the list. #CostVR
People may not realize that for some meds in the US, the money you pay as insurance copay/coinsurance may be higher than what you can get paying out of pocket without using insurance.

It's one of the mind boggling things about prescription drug prices.
Not even people in healthcare know.

A friend who works in healthcare commented that after I told him about the way meds are priced, he's now saving $1200 a year on just one medicine.

At a list price of ~$19,000 a month, I will not be using or recommending much of melphalan flufenamide (Pepaxto) for multiple myeloma.

The drug needs to show superiority over cyclophosphamide or melphalan in a randomized trial. #CostVR
Myeloma drug costs are unsustainable. They reflect what's going on in all cancers. If key opinion leaders are silent this will continue. Our recommendations must reflect and take into account value.
I said the same when selinexor and panobinostat were approved. Pricing has to be related to incremental value.

Forget the pharma talking point that high prices are needed for innovation. If every drug can be priced high regardless of value there is no incentive for innovation.

My paper on the high cost of prescription drugs and possible solutions is out. I go into how US policies (or lack thereof) affect prices in other countries. #BCJ
#OpenAccess #CostVR #insulin4all @MayoClinic nature.com/articles/s4140…
There are no easy answers because the factors contributing to high prices are myriad, and come from all different directions. There are factors unique to the US, which are not operational elsewhere. We need many policy changes in order to have an impact. nature.com/articles/s4140…
Looks like a laundry list of things going on, because a laundry list of things is going on. Everything I mention plays an active role in high prices. We cannot control prices without looking at this 360°

There are things individual docs can do. So don’t wait for policy changes.
