I feel Twitter is the best social media platform. It has tremendous social, educational and networking value.

Personally it has helped me create a lot of awareness about the importance of oral health and clear a lot of myths about my profession.

As an aside, it has enabled me
to help people in dental pain also.

What happened today was the best example of that.

I woke up to a few people tagging me in this tweet:

To start this weeks #FluorideFriday session, we talk bout overfeeds, accidental poisoning and #fluoridation.
The #StannousFoilHat wearing types do not like to talk about this issue. Never have, either. This from the infamous 1951 meeting.

"The barrel wouldn't fall through."
However, overfeeds are a common issue, along with underfeeds. This Spring it happened again.

Have no fear, though, it is only a '*cosmetic*' dental problem.

Heartening to know that today there are gobs & scads of young #Dentist's who want to learn more about who precisely WAS "C.C. Bass, M.D", and what did he have to say about #PreventiveDentistry?
For the last few decades or so, just about any young #Dentist, when they first learn the real story of who "C. C. Bass, M.D." is, and what he actually did, are puzzled.
A couple of channels results from their #DentalSkool teaching, and thus their thinking.

Every week, I get at least one phone call complaining of severe toothache, almost always in the molars. (Back teeth).

The patient comes in and the clinical examination and xrays show no cavities, and the tests for fracture are usually negative at this stage.

#Pune #Dentist
Usually it can only mean one of two things:
1) If the pain is restricted to the upper molar, then a recent of concurrent history of sinusitis/runny nose/cold etc. I typically see this around late November and the history is very clear.

2) BRUXISM!
What is Bruxism?

In plain and simple words, severe uncontrolled undesirable grinding of teeth against each other, usually occurs when the patients is asleep and the patient has no idea they are doing this.

The test is very simple. I will use my finger to test the Lateral

“Doctor why do I need to take this letter to my physician? It’s just a tooth l! Just take it out! What does my heart have to do with it?”

If you are a dentist, you have heard this line many many times!

Here’s why:

Old family friend.

Wife passed away many years ago due to
Cancer!

Son in the US!

Not much going on for him other than a job!

Over the last few years, ended up neglecting his health!

His diabetes was through the roof!

The 3 month average, HbA1C, was around 10 for years! (Even for diabetics it should be under 7)
Tried very hard to motivate him to address it! No change!

Wears an upper complete denture and has a few broken teeth in the lower jaw!

Met someone who got a full mouth implant rehabilitation done and so now wanted the same!

FINALLY got it to 7.5ish in December!

Great!

I wonder whose bright idea it was that if a #vagina managed penetrative #sexual activity, it must be pliable to #insertion of the speculum 😒. Whoever that was definitely disregarded the physiology of sexual #arousal & vaginal changes in the moment that facilitate #penetration.
@vagina_museum shows that everyone's apparatus is different then how can #Gynecology stick to a one size fits all model? #publichealth should strive to develop minimally invasive self administered #screening tests & #gynaecologist must #research on making procedures comfortable.
researchgate.net/publication/22…
This shows that topical #anesthetic agents reduce #pain and #discomfort during speculum manipulation. Why can't the usage of topical #anesthesia be a #standard process in #Gynecology? Why such blatant disregard for #patient #comfort? 😠 #misogyny much?

My 2658956th post on why you should see a dentist on time!

This patient had been advised to get her wisdom tooth removed on time. Out of fear, she never got it done!

I removed another tooth for her on the other side and she had no issues at all!

So she knew it won’t hurt!
She finally called 2 days ago saying she is in a lot of pain and needs the tooth extracted!

The issue was that the tooth was very close to the nerve and there was a chance of permanent nerve numbness if it were to get damaged during extraction.

You might say then why take it
out?

Because it was decayed and there was a bigger chance of nerve numbness if the tooth got infected and it spread into the nerve!

That’s exactly what happened!

The right half of her chin and lip was “tingling” when she came in today!

The infection was pressing on the

I feel there are two surgical procedures that have changed people's lives for the better more than anything else in the last 2 decades.

One is total knee replacements. I remember watching so many Aji Ajobas struggle to just move from one room to another as a kid because of bad
knees.

Now I see so many people in the 60-80 age group climb stairs and walk on the Deccan Gymkhana ground effortlessly thanks to their new knees!

The other is dental implants. I don't think there is adequate awareness of how life changing they are and hence this thread.
For starters, I would like to share something I have observed over the last one year.

It is sad but true that most "Mothers" in the 60-80 age group are the epitome of sacrifice.

More than anything else their personal health has been neglected.Specifically their dental status!

How about a #FlashbackFriday, also a #FluorideFriday #FluorideFact. Dr Dean did his level best to slow the bandwagon down, to no avail. A very specific, small population mentioned.
That paper is the cornerstone of the theory that adding #fluorine to our diet using our water prevents tooth decay.
The stone is probably fluorite...
Speaking of stone, and #fluoridation, and putting things on pedestals, DYK?

#Thread to discuss medical clearance for dental treatment:

A common misconceptions about dental treatment is that it is not affected by a patient's medical history or vice versa.

"Aho daat tar aahe. Tyacha majhya heart attack/knee replacement/diabetes shi kay ghena"
"Its just teeth. What is the connection with a heart attack/knee replacement/diabetes?"

And then there is the issue of getting medical clearance in writing from their specialist doctor.

I will try to cover most common conditions, associated risks and desired clearance!
1) Diabetes:

Where there is sugar, there are bacteria.

Where there is bacteria, there is infection.

Plain and simple.

So a patient with uncontrolled diabetes is a much increased risk for infection.

What is needed:
1) Current labs. Blood Sugar and HbA1C less than 120 days

Serving up some hot cognitive dissonance here. -|- "Although dentistry has long been identified as being far more preventive-oriented than most other areas of medicine, the emphasis on disease treatment is still the predominant activity of most dentists."
The problem with #Dentistry and #Prevention in a nutshell. -|- "In contrast, if the dentist is accomplishing the preventive procedures, the overhead is enormous, and the practice may not have adequate revenue."
Ditto. Treatment ≠ Prevention, no matter how you phrase it. -|- "What preventive orientation can the dentist have? The following procedures can provide preventive characteristics to your restorative treatment."

#Hi everyone, I am a #dentist. I have been following the #pandemic and #economy very closely. I have been posting stories on #COVID-19 on my Instagram and YouTube. I will now add my #thoughts and #research here.
The #rateofspread is very concerning. If #dental personel were worried in March/April, #July could be much worse.
We should not be too concerned about #aerosols; we need to be concerned about #communityspread. If July results in a #outbreak, #dentaloffices will be more at risk than ever, just from the odds of more infections in the population.

#Thread: "Are dentists doctors?"

This question comes up time and again, directly or indirectly. I don't know why but it does.

So I posted this poll and the results surprised me. The popular joke was that there were more dentists voting given the results!

#dentist #doctors
I am a dentist. BY CHOICE. I got into Mechanical Engineering but moved to dentistry by choice. Could have gotten into a government medical college but was never interested in Medicine.

I studied in an amazing institute here and in the US, was able to practice and teach there.
Now back in Pune. And I LOVE my profession.

Which brings us to the basic question "Are dentists doctors?"

And the answer is Yes AND No! and it does not end with being a doctor. We are much more than just that. Let's see how.

#Thread about brushing your teeth:

When I came back from the US in late 2018, one of my goals was to create awareness about oral health. I was very surprised to hear how many people don't know its essential to brush multiple times.

#oralhealth #preventivecare #brushyourteeth
So I created this poll and the results confirmed my fear. More than 50% said they brush ONCE a day! So here goes.

Teeth can get two main diseases: Caries/cavities and periodontal/gum disease. Both are multi factorial but start with two main factors: Diet and oral hygiene.
Genes play a role but let's not blame our parents and their parents for our faults.

Basically food left on teeth causes caries and under the gums causes periodontal issues. Plain and simple. Some foods are worse than others but oral hygiene is key. So lets start with brushing.

I dove into the FEC data, and it's pretty clear:

Grassroots health professional donors far prefer Sanders and Warren to Biden and Buttigieg. 1/
For doctors, donations went mostly to Warren and Sanders, with Sanders slightly receiving more.

Buttigieg received a substantial portion.

Biden received very little.

#MedTwitter #MedStudentTwitter
@darakass @DrDooleyMD @AbdulElSayed @DrSriram @DoctorsforPete @Docs_4_Bernie To me this corroborates the polls which show that while medical trade groups/guilds/professional societies might not support policies like #MedicareForAll, grassroots doctors feel differently. 3/

October is Breast Cancer Awareness month.

I wish to update everyone on the role we play in early detection and treatment for women affected by this terrible disease.

#breastcancer #October #breastcancerawareness #Pune #dentist #cancerscreening
As part of my training at the University of Pennsylvania, we were taught the role of regular Oral Cancer Screening to be carried out at each visit.

The mandible is the most common site for metastasis aka secondary spread of Breast Cancer.
As such we contribute both towards primary diagnosis and ruling out recurrence. A simple panoramic xray is generally sufficient in most cases.

If a patient is to undergo Chemotherapy, it becomes very important to eliminate all potential sources of infection.
