🧵Great review of the #340B program, how it has veered away from its intent, and negative consequences for #DrugPrices. | ⁦@wasan_kumar⁩ ⁦@kevin_schulman

Some takeaways follow 👇 ImageImageImage
2/ “The 340B program was established in response to concerns among safety-net providers regarding the Medicaid Drug Rebate Program, which was enacted in 1990 and required manufacturers to offer their lowest prices to the Medicaid program.”
3/ “An inadvertent effect of [Medicaid Drug Rebate] policy was that safety-net providers lost access to the discounts they had previously received from drug manufacturers. The 340B program was intended to restore discounts to providers caring for low-income and uninsured people.”

While @PhrMA wants you to think companies raise #drugprices to fund R&D, our investigation revealed that drug companies raise prices to:

💰 Turbocharge sales
💰 Keep their stock price growing

Here’s an internal company document admitting they raise prices to fuel “growth”:
Worse still, drug companies spend far more to reward shareholders than on innovative R&D.

(Read our staff report on buybacks and dividends here: bit.ly/3pGmkR)

Here is the money they make vs. what they spend on R&D for 7 drugs:
Our investigation also found that drug companies hike prices for life-saving medications because the U.S. allows them to get away with it.

Here’s an internal document admitting US policy lets the drug company Teva “successfully” raise prices:

1/ Dec. 18, ⁦@AmerMedicalAssn⁩ wrote to ⁦@HHSGov⁩ urging it to withdraw the recently issued Most Favored Nation (MFN) Model interim final rule (IFR): tinyurl.com/yajlvlb7

Issue has broad impact on patients, e.g. those who have #cancer, GI, #rheumatology disorders.
2/ This IFR would:

•Impact pricing and availability of many of the most expensive Medicare drugs nationwide

•Have serious detrimental impacts on patient access to needed medications

•Create strain and uncertainty for physician practices
3/ @AmerMedicalAssn has long supported efforts to address escalating prescription #drugprices and reduce financial burdens on patients.

However, the MFN IFR is deeply flawed.

The U.S. surpassed 350,000 reported deaths from #COVID19 last night.

And Trump, Pence & Alex Azar are doing what about it today?

Where is the accountability for these deaths?

As of yesterday #CDC says 13,071,925 doses of #COVID19 #vaccines distributed but only 4,225,756 administered.

#FDA's #VRBPAC doesn't meet until Dec. 10 on $PFE/ $BNTX & Dec. 17 for $MRNA/#NIH vaccines. Does Meadows think he can really push FDA to move the dates up sooner?
What is Meadows & Trump doing to ensure states have enough funding to actually distribute #COVID19 #vaccines?
Maybe Topol needs to ask his friend @SteveFDA if that meeting went so well yesterday & Hahn stood up for #FDA then, why is Meadows & Hahn doing it all over again today?

#COVID19 #vaccines
It was just last week when @SecAzar insisted the "proof was in the pudding" about Trump & pressuring #FDA.

So, two trips in two days to the White House for @SteveFDA.

What kind of pudding are they making at the White House @SecAzar?

#CDC has scheduled an emergency meeting of #ACIP for Dec. 1 to vote on which groups of Americans should receive the limited initial doses of the #COVID19 #vaccines once those products are cleared for the U.S. market.
On Dec. 1, #CDC's #ACIP is expected to vote on whether the 21 million U.S. healthcare personnel & 1.3 million residents of long-term care facilities should be first in line for the #COVID19 shots — phase 1a of the vaccination program.
If both the $PFE - $BNTX and $MRNA-NIAID #COVID19 #vaccines are granted EUAs in December, there are expected to be only enough doses by the end of 2020 to vaccinate about 20 million people in the U.S., with more doses not coming for the other 310 million Americans until later.

If @ABC is still paying @ChrisChristie to be a contributor/commentator, it needs to stop it now.

Christie at White House right now at yet another taxpayer-paid political event, where Trump is making a number of false statements, providing no evidence whatsoever of his claims.
And news reporters shouldn't be participating in these types of taxpayer-paid political events at the White House, where they sit quietly by as Trump makes a number of false statements, providing no evidence whatsoever to back up his claims.
This is not a news conference. It is a taxpayer-paid political event, where Trump is using the public airwaves/news publications to spread false/misleading information.

Why are news reporters participating in these taxpayer-paid political events?

More allegations emerged that political appointees in the Trump administration have been attempting to interfere with and impose influence on the scientific work of federal agencies to downplay the severity of #COVID19, which has killed nearly 200,000 Americans.

Meanwhile, after waiting seven weeks for Trump to reveal the text of his so-called most favored nations executive order for the Medicare Part B program, he revoked the July 24 order on Sept. 13 and replaced it with an amended version, seeking to include the Part D program.
Executive orders simply instruct federal agencies to take certain actions. But the orders have limited authority, can be difficult to enforce without legislation, take months, if not years, to implement and can easily be challenged in court.

Honestly, Moderna is an expert at grifting during the #COVID19 pandemic.

This is what every rational government should be watching out for:

Step 1. Convince the US to invest $1.5 billion in your vaccine development, despite no record of vax marketing.

Step 2. Make sure you bolster and research a vaccine the government likely ALREADY owns.

PBMs: No
Some tension rn btw Express Scripts&Pfizer reps

Pfizer: It's too difficult to get biosimilars on formularies "bc of rebate strategies" 4 original biologic

ES: Disagrees. Biosimilars must add value/cost less. To drugmakers: consider that when they’re launching their list prices
