So what are #PBMs? Chances are, if your meds suddenly got changed by your insurer, or you’ve been waiting weeks on #priorauth for a med you need, or you pay $$$$$ for a drug in the US that is cheap overseas, you’ve already experienced a #PBM and didn’t even know… twitter.com/i/web/status/1…
That’s b/c #PBMs aren’t meant to be known, they’re meant to be a middleman in healthcare cutting in an extra layer of profit for them and an extra cost to you…wonder why #prescriptiondrug pricing makes no sense compared to prices in other countries? This is why.
Pharmacy Benefit Managers (PBMs) are third party entities paid by healthcare insurance companies to manage all things meds: the formulary (what is/isn’t covered), the price from manufacturer, and the cost (copay or “tier”) to the patient.

As promised, a 🧵 on GoodRx.

Starting off with the basics…

What is it?
GoodRx is a prescription drug price tracking and discount company. It’s essentially another middle man in the messy landscape that is #drugpricing in the US.

But it’s more complicated than that… #TwitteRx
GoodRx is best known for their coupons. They track prescription drug prices and offer patients a discounted price, which varies a bit based on which pharmacy you select.

Here’s an example of what a search for enalapril looks like…
2/17
GoodRx coupons CANNOT be combined with insurance. ❌

Now, this *might* be helpful for a patient who is completely uninsured, or someone on relatively few inexpensive meds who never hits their deductible anyway, but even then, if you ask your pharmacist for a cash price instead…

#CDC has scheduled an emergency meeting of #ACIP for Dec. 1 to vote on which groups of Americans should receive the limited initial doses of the #COVID19 #vaccines once those products are cleared for the U.S. market.
On Dec. 1, #CDC's #ACIP is expected to vote on whether the 21 million U.S. healthcare personnel & 1.3 million residents of long-term care facilities should be first in line for the #COVID19 shots — phase 1a of the vaccination program.
If both the $PFE - $BNTX and $MRNA-NIAID #COVID19 #vaccines are granted EUAs in December, there are expected to be only enough doses by the end of 2020 to vaccinate about 20 million people in the U.S., with more doses not coming for the other 310 million Americans until later.

So #drugpricing twitter is going to be all abuzz about SCOTUS discussing PBMs. But let's be clear - this case isn't about drug prices and PBMs in the way we usually think about them. 1/7
statnews.com/pharmalot/2020…
Instead, it's about whether the state can mandate higher payments for less competitive pharmacies. The community pharmacy lobby complained that they weren't getting paid enough, and the state put in a law to protect them. 2/7
Community pharmacies can't purchase generic drugs at the same extreme discounts as larger purchasers. When PBMs use the average acquisition cost of drugs to reimburse pharmacies, that average can be below what community pharmacies paid to buy the drugs. 3/7

More allegations emerged that political appointees in the Trump administration have been attempting to interfere with and impose influence on the scientific work of federal agencies to downplay the severity of #COVID19, which has killed nearly 200,000 Americans.

#CDC #FDA #NIH
Meanwhile, after waiting seven weeks for Trump to reveal the text of his so-called most favored nations executive order for the Medicare Part B program, he revoked the July 24 order on Sept. 13 and replaced it with an amended version, seeking to include the Part D program.
Executive orders simply instruct federal agencies to take certain actions. But the orders have limited authority, can be difficult to enforce without legislation, take months, if not years, to implement and can easily be challenged in court.

Daniel Best, who worked for HHS, and helped lead the effor to lower prescription drug prices, was found dead around an apt complex garage. He died of “multiple blunt force injuries” but it was ruled a suicide. WTF?!?!

So, I looked up "suicide by multiple blunt force injuries" & came upon this medical article of an example. It's rare.

This guy (in link) had head injury, cuts, & abrasions all over his body, but also paranoid schizophrenic w/ previous suicide attempt.

ncbi.nlm.nih.gov/pubmed/12464812
There's another brief example in here, and that person also had a history of schizophrenia.
