Like many, I’ve had some extra fee time these days... After rebinging @theofficenbc and #theofficereunion I was inspired.

A thread of Trump’s handling of the #coronavirus (#COVID19) pandemic, as told by Michael Scott. #TheOffice
Way back in 2018, Trump decided it was a good idea to disassemble the #Pandemic Response Team – created by the Obama administration in case there was “an airborne disease that is deadly.” This can’t go wrong! #thiswaspreventable
Trump does nothing to replace this. He plays golf and insults people on Twitter – just the normal things that presidents do right?

1/11This weekend sucked. I swabbed a patient for Covid today (one of many). She was convinced she didn’t have “it” because she’d been swabbed two days before & it was negative, despite all of her symptoms pointing to it. The swab is awful & we are seeing 30-40% failure rate.
2/11Because if not done correctly (which is fucking awful) you get ‘negative’ tests. So I swab pretty aggressively. I made her nose bleed unfortunately and she cried. I felt terrible about that. Her test came back positive. She broke down & cried. I sat with her in my PPE
3/11where it’s impossible to really connect. She asked “can you promise me I’m not going to die?” I didn’t know what to say. I said “your numbers look good and most people do okay with this. She then asked “what are you going to do here for me?” I explained the plan

We still had time to get things right - we needed 20 sets of #PPE per patient per day- the Govt was told this, & when #COVID19 first appeared at the start of February @BorisJohnson & his govt did nothing
The Govt has delivered through the whole crisis only 1,358,518 gowns but the #NHS needs 100,000s a day, when panic set in they used spin to say ‘we followed the scorn so a total utter lie & criminal negligence
Many #frontline staff are angry at the Govt, one ICU nurse said calling us heroes just makes it ok for us to die

1/ #Frontline clinicians across the world are observing a #hypercoagulable state in patients with #COVID19. Could the severe and varied effects of the disease on multiple organ systems possibly be due to thrombus/micro-thrombus and associated sequela? @ASH_hematology @EricTopol
2/ Could a mechanism of #microvascular injury during the infection be through inappropriate effects of excess free angiotensin-II protein floating around because the ACE-2 that would normally be soaking it up is occupied by #SARSCOV2 particles? @EricTopol @ASH_hematology
3/ We know that ACE2 activation promotes antithrombotic activity and ACE2 deactivation would promote thrombus. #medtwitter #NurseTwitter #COVIDー19

#medtwitter and everyone. Pls read this thread. I want ppl to know what those in #nursinghomes and #PALTC med are doing, without the recognition. For every person u save from #COVID inpatient, please know, we're working non-stop to help you, and it's taking its toll. 1/
As early as 3/1 we've shut down visitors, tried to mitigate spread. But we're not the hospital and have less resources. And hospitals rely on us to d/c patients who can't go home. We're taking your pts, knowing full well they may bee carriers of #COVID but testing was not done.2/
Simultaneously we've been forcing the hard conversations, trying to get patients/surrogates to understand the futility in #fullcode status + even hospitalization right now. Meanwhile families have not been allowed to visit in 4 wks +. Despite r efforts, #COVID is in our own 3/

@billy_blog lovely to meet you, @wbmosler and Martin yesterday.

I hope your journeys went well?

A couple of slightly #tangential things from the #frontline I think you'll find of interest;

You may be amused at the rankings for #CivilService for the UK, The US and, say, #Germany...
