Those UK doctors who moved to my Aussie ED realised we never had an Exit Interview. We never got to share some of the examples of NHS culture that drove us away. Perhaps we can inform you a little with some anecdotes? 🧵
If you're interested, i've already done a thread on the employment conditions that enticed us to stay in Australia.



This however is the experience of 10 UK trained docs (now Aussie EM Registrars) and the NHS culture that drove us away.
If the owners of the anecdotes have consented i've tagged the relevant trust so they can consider their own trust culture.

We're not alone in UK #MedTwitter in our experiences (as @Xeon4f145d96s1 posts show) but here's just a sampling of why it was so shit in the NHS

There are so many issues with this article which seems to have been framed to fit the government agenda.

Disappointing because we were explicit about a number of things.

"No junior doctor has seen pay cut by 26% in that period".

We are comparing ourselves now to the same role in 2008. e.g. F1 to F1.

No doctor is worth less than they were in 2008.

The pay uplifts awarded by DDRB are across Junior Doctors just like inflation affects all Doctors
"they move up the pay scale over time until they finish their training."

We were at pains to describe that this is not automatic. There are artificial bottlenecks. Doctors also have to demonstrate significant competencies including passing extremely difficult expensive exams.

Doctors are furious.

A record breaking turn out of 77.49% and a 98% yes mandate smashing the anti-trade union legislation that demands POSTAL votes for a significant size of the workforce who rotates on an at least an annual basis is undeniable.
What is more is that we told everyone our 72 hour full walk out first round of action back on 6th January.

Doctors voted for this with their eyes open.

Doctors are furious.

@SteveBarclay should have seen what was coming.
We went through hell and back in covid.
Every winter has gotten worse and worse and worse.
And yet NOTHING changes.

This is what happens when a government doesn't listen.

To clear things up for the anti-junior doctor brigade:

1. We are not allowed to do private work until we are consultants.

2. We fund our mandatory exams and professional fees/registration ourselves.
3. We have a significant amount of responsibility at ALL stages of post grad work, we deserve appropriate pay for it.

4. Things change over time. We can never know how bad things are from the outside, even whilst applying for or at medical school.
5. If we all leave in search of better pay and conditions, who will staff the NHS? I am sure those criticising doctors for wanting better pay will be the first to complain when there's nobody left to treat them.

I think it's useful to note that in an environment in which trusts are directly employing insufficient numbers of staff on unacceptable rates of pay; they need to consider non-pay factors which they can affect... 🧵
Transport/Parking: Paying to park at work is an additional tax on working that we resent. As potentially the largest employer in the local area what do you know about how staff get to work and why?
Do you have safe, reliable, green transport options which are acceptable to staff?
Food/fluid: Are there free tea/coffee/squash/cold water in your staff rooms?
Are there nutritious, delicious, cheaply-priced/free food options available for all your staff at all times of day and night?
