#FuneralHomes were once dominated by local, family owned businesses. Today, your local funeral home is likely owned by #ServiceCorporationInternational, who bought hundreds of funeral homes (keeping the proprietor's name over the door), hiking prices and reaping vast profits. 1/ An altered version of Goya'...

What were we saying? Oh yeah: turkey day.

Friends, family, & loved ones up & down the state want to spend this special time of the year together. But that means travel. And so also carbon emissions. (4/14)
Ex: a 2022 AAA study said >54 million would travel for T-Day in the US, & >90% would do so by car.

Even accounting for carpools, that’s a cornucopia of carbon intensive VMT. [1]

Here are some comparisons to so the carbon cost is more relatable. (5/14)

SF<>LA Round Trip:

Flying:

~390 lbs. of CO2. [2]

Driving 802 Miles:

~497 lbs. [3]

Some estimates of emissions of turkey, stuffing, potatoes, cranberry sauce, & pie for 12 guests:

~103 lbs. [4]

1 single trip above is 3 times or more emissions than a 12-person meal! (6/14) Detailed description of thi...

You know what’s super wonky? VMT. That’s Vehicle Miles Traveled.

Today, right before turkey day, involves A LOT of VMT.

Ex: We’ve all seen the sea of red & white lights on I-405!

Did you know #HSR has proved around the world it can help?

Join us for #WonkWednesday! 🧵(1/14)
Let’s start at the beginning: what on earth is VMT?

In brief: it is the number of miles travelled by vehicles. If 1 person travels 1 mile in car alone? That is 1 VMT.

If 255 people travel 1 mile in #HSR? That is *also* 1 VMT.

(Please know, this is simplified.) (2/14) Density of people moved in ...
Crucially, because the transportation sector is such a large contributor to California’s climate change causing greenhouse gas emissions (well over 40%), reducing VMT in California is a necessary part of the fight to protect our planet. (3/14)

New study in #HSR @WileyHealth from Danielle R. Gartner PhD @WhitneyEpi et al investigate the intersection of race and economic context in treatment with hysterectomy among reproductive aged women with noncancerous gynecologic conditions.
1/4
@WileyHealth @WhitneyEpi WHAT IS KNOWN Historically, treatment with hysterectomy has been highly geographically variable. Despite an overall reduction over time, rates of hysterectomy remain higher among black women than among white women
2/4
@WileyHealth @WhitneyEpi WHAT THIS STUDY ADDS Black-white rate differences (ie, disparities) are wider in areas with higher socioeconomic status.
Medical strategies to reduce hysterectomy may have been inequitably implemented.

New study from @ThomRapp @momotazur et al in #HSR @WileyHealth examine whether stronger referral relationships between hospitals and skilled nursing facilities (SNF) are associated with lower-risk patients being admitted to SNF
1/4
@ThomRapp @momotazur @WileyHealth WHAT IS KNOWN: Evolving payment models are changing the relationships between hospitals and skilled nursing facilities.
Hospitals concentrating referrals to SNFs have exhibited better outcomes (eg, lower readmissions). No study has been done from the SNF perspective.
2/4
@ThomRapp @momotazur @WileyHealth WHAT THIS STUDY ADDS: SNFs with higher concentrations of hospital referrals are associated with less-risky patients at admission. Certain patients may be facing more restrictive choices of nursing homes at discharge.

New article in #HSR @WileyHealth by @afrakt assess the prevalence of patient administrative tasks and whether they are associated with delayed and/or foregone care. ABSTRACT: hsr.org/node/680108
1/6
@WileyHealth @afrakt WHAT IS KNOWN: High administrative complexity is a central feature of the US health care system, with under-appreciated implications for patients. Research examining the administrative costs of health care has primarily focused on clinicians and provider organizations.
2/6
@WileyHealth @afrakt Compared to financial barriers to access, evidence examining nonfinancial costs has been limited and not focused specifically on patients' administrative burden.

Neeraj Sood PhD et al look to understand the effects of receiving vertically integrated care in inpatient rehabilitation facilities (IRFs) on health care use and outcomes in #HSR @WileyHealth article. ABSTRACT: hsr.org/node/680121
1/5
@WileyHealth WHAT IS KNOWN: Coordinating care during transitions from acute to postacute care may be easier to accomplish when providers are vertically integrated.
2/5
@WileyHealth Prior studies have mainly focused on effects of vertical integration between hospitals and SNFs for fee for service Medicare beneficiaries and found mixed results.

Shih-Chuan Chou and colleagues examine the association between #highdeductible health plans and #EDvisits with low-value imaging in this #HSR @Wiley_Health study. Thread below: (1/7) hsr.org/node/678800
WHAT WE KNOW: Health insurers increasingly offer high-deductible insurance products in hopes of improving care value by giving patients “skin in the game.” (2/7)
Past studies showed that high deductibles reduce outpatient medical visits or tests, which are primarily under patient control, such as attending appointments, cancer screening, or ED visits. (3/7)

Lee Revere Ph.D., et. al. evaluate the robustness of the CMS Hospital Value-Based Purchasing (HVBP) total performance score (TPS) in new #HSR article @wileyhealth Abstract: hsr.org/node/677720
1/4
@WileyHealth WHAT IS KNOWN: TPS is intended to be capable of differentiating hospital-level quality on an annual basis. Prior research found evidence that highest performing hospitals maintain their status year after year while preventing lower-ranked hospitals from increasing in position
2/4
@WileyHealth FINDINGS: Annual TPS is skewed, showing a large gap between top-performing hospitals and all others. Results also show significant movement year to year, with hospitals moving in and out of the top and bottom performance categories disproportionately

🧵 Today marks 10 years of working on @CAHSRA for me. It was supposed to be a 6-month assignment but here we are. After 10 years I believe in this project more than ever but want to share some of what I’ve learned along the way. (1/n)
There’s a great quote from Bill Gates:

“We always overestimate the change that will occur in the next two years and underestimate the change that will occur in the next ten.”
(2/n)
10 years ago, this project was recently endorsed by the voters of California and had received federal funding, but was little more than lines on a map and the vision of what kind of system we wanted to see in California. That vision was the right one. It still is. (3/n)

Not only experience, which is important, but clinical #research does not parse out the Japanese man or woman from the South Asian man or woman on heart disease risk, epidemiology, treatment.

#EBM lacking for us then

And then there is the question of who is white.

Iranian/Persian & Arabs are white on census.
Does the rest of America agree?

Racists & xenophobes tend to use loophole to say being anti-Muslim (whom they consider to all be Arab) is “not racist.”

latimes.com/projects/la-me…
Proving again: race is a social construct

Not until 2020 Persians really started to call selves “POC” (for self protection)

Have lots of Persian friends
Urdu culture has Farsi roots

But let’s be honest: Persians have seen selves as superior race, apart

Let's go! 1/
1) When engaging in thoughtful critique, whether your own or someone else's, read the entire paper & supplement.
1x) Critiques of Greenwood et al that raise issues such as SES, comorbidities, etc, imply that the commenter has not read the entire paper. Specificity is key.

2) When identifying limitations of the methodology, differentiate between a) those not mentioned by the authors; b) those mentioned by the authors but not addressed as fully as desired.

Important notice. Do NOT make the mistake of buying an Apple product, and if you own one definitely DO NOT take it to iCare for repair.

A THREAD

Please do read this as I'm getting no help from any official channels, and now forced to turn to social media.

Attached later will be some photos of the issue, my bill and so forth.

The initial issue:

On 28th December 2019, I decided to try to dive into the Apple ecosystem, and bought myself the iPhone 11 Pro from
@RelianceDigital , with a 1 year extended warranty.

2
The initial issue:

On reaching home an issue I noticed was that my phone would not charge beyond 80%. There is a setting to turn off "Optimised Battery Charging" which should allow the battery to charge to 100% everytime when turned off regardless of anything.
