I don’t believe many of you understand how bad our Covid-19 situation is becoming here in the Treasure Valley. I wanted to share my experience as a family doc these past few days so you can see why I am so concerned. 🧵
Saw 3 patients yesterday with Covid. One with high risk of progression to severe disease (overweight, high blood pressure, asthma) who needs Regen-cov. Cannot schedule infusion for another 3 days due to patient demand. Very worried he will be in hospital by then😔
Multiple requests for ivermectin in past week. A lot of scared people out there. Sorry Ivermectin is NOT EFFECTIVE. Masks and the vax are VERY EFFECTIVE!

IDHW Director Jeppesen: 93 percent of Idahoans who got their first dose have received second dose, and 4.9 percent are within the time frame to get their second dose. #idahocovid19
Idaho is now receiving a bit more than 50,000 first doses of Pfizer and Moderna a week. This week, Idaho also received 2,500 doses of J&J.
NEW: High-risk Idahoans ages 16-44 will be eligible to get the vaccine beginning April 12th

The motion to adjourn tonight's meeting came during a presentation from Dr. David Peterman of Primary Health, who was talking about an 18-year-old patient who now needs a pacemaker after getting COVID. #idahocovid19
Before that, he was telling the board about COVID ripple effects, such a two-year-old patient with speech delays. COVID is affecting Peterman's ability to get therapy for that child -- something that could have devastating affects for their long-term development. #idahocovid19
This comes the same day as we broke 2,000 new cases in IDHW's daily tally.

All of that, plus hospital updates we didn't get to hear presented to CDH, will be eclipsed by news of the protests.

That doesn't mean the problem is going away. #idahocovid19

You can listen in on the AARP Idaho call with Gov. Brad Little here: facebook.com/AARPIdaho/ #idpol #idahocovid19
Gov. Little on healthcare capacity: "It is concerning in certain areas." (I think some hospital CEOs would say this is an understatement.) Predicts because of the vaccine, Idaho can go relatively back to normal in late April, May, June-ish.
IDHW Director Jeppesen says if the Pfizer vaccine is approved this weekend as expected, Idaho will likely receive its first shipment of doses on Tuesday or Wednesday. #idahocovid19

Listening to the CDH board meeting, and there are protesters in the building interrupting the director as he's trying to speak. Watch here.
Dr. Jim Souza says while hospital capacity is tough, the real story is staffing shortages.

"We are definitely employing what you would call contingency standards of care.” Not crisis, but “not adhering to our usual modus operandi.” They almost went on divert earlier this week.
St. Lukes "will move a mountain before we go on divert," Souza says, but they were almost there this week. "What spared us were some unanticipated deaths." #idahocovid19

A friend sent me this letter Idaho’s Lt. Governor and other leaders have signed against any action @CDHidaho might take tonight to protect our community.
Just a reminder that while some people may think that life just needs to continue as normal without taking steps to protect one another, there are people in Idaho DYING every day and exhausted #healthcareheroes sacrificing and doing everything they can to try to save others.
Thank you, @TransitGeekJoe.

Good morning. I'm listening to @boisemayor McLean's briefing from local health care leaders about the current healthcare situation in the Treasure Valley.
@boisemayor St. Luke's Dr. Souza: We are now well beyond normal staffing operations and approaching the limits of what we can do. He reiterates what we've been hearing for a while: We're getting closer and closer to rationing care, which is entirely preventable. #IdahoCovid19
Souza suggest enforcement and consequences to those in the business community who aren't adhering to existing mandates. (So many officials have shied away from this.)

After more than three hours of (around 60) public comments, the Twin Falls City Council decided to indefinitely table a discussion on the face mask mandate. #IdahoCovid19
The motion to table was made by Shawn Barigar, former mayor, and the councilmember that suggested the mask mandate discussion.

Barigar is now bargaining with the man who organized a pre-meeting protest.
The councilmembers are talking about the need for education to an audience that spent three hours telling them they don't believe in masks.

Happy Thursday afternoon! I'm watching the Panhandle Health District Board of Health meeting (my fourth BOH meeting this week!!) Updates here. #idahocovid19
Jeff Lee from Kootenai Health: "This is going to be short, bu I can't guarantee sweet." Lee says cases are surpassing peaks from July. I can't see the slides on this stream, but Lee says they may quickly outstrip hospital's availability. #idahocovid19
Lee says his slide is already out of date. "We've had three more deaths since I updated this an hour ago." #idahocovid19

Grim updates from St. Luke's Magic Valley, Cassia, and Minidoka hospitals. Minidoka and Cassia are holding, but have very high positivity rates (30 to 40 percent). St. Luke's Magic Valley has no more room. All are reporting critical staffing shortages. #idahocovid19
Dr. Joshua Kern of St. Lukes MV: "At no point during the pandemic so far have you heard St Luke's crying wolf.... Today is the day." Hospitals are at capacity, hospitalizations are a lagging indicator, and cases continue to rise. Next two weeks could be very bad. #idahocovid19
Administrator from St. Luke's MV is speaking now, saying suggestions and resolutions on masks, as opposed to mandates, aren't working, and puts the burden on frontline workers. "Not making decisions to impose things that we know will work will ultimately shut down the economy."

Listening in on this. Dr. Nemerson points out that this spike that we're experiencing isn't starting from zero -- we're already at a moderate prevalence of the disease in our community, and rising.

The current test positivity at St. Alphonsus is 12 percent. #idahocovid19
Joshua Kern, St. Lukes Magic Valley: All counties in the area are simultaneously experiencing an increase in cases right now. More than 40 COVID patients hospitalized at the beginning of the week, which was more than 1/4 of all hospitalized patients there at the time.
That was a record number of COVID patients for St. Lukes Magic Valley.

Meanwhile, hospitalizations are a lagging indicator, and cases aren't going down, Kern says. They're bracing for more.

They also have their first confirmed second infection in an #IdahoCovid19 patient.

Sparse crowd for the COVID skeptic rally in Twin Falls today organized by the John Birch Society and featuring Lt Gov McGeachin and at least 7 GOP lawmakers. #idpol #idleg #COVID19 #idahoCovid19
Guys with ear pieces wearing III% militia gear appear to be providing security. And Idaho III% president Eric Parker was scheduled to speak, although not seeing him as of yet.
Almost no masks, lots of hugs and handshakes, no distancing as Idaho’s. COVID-19 cases continue to spike.

For those listening to Panhandle Health District, their stream went down. Their PIO said they're trying to get the county to fix it. #idpol #idahocovid19
Watching the KHQ feed, which is nice, but doesn't fulfill the open meeting obligations. I believe they are stopping the meeting right now to try to fix the Internet issues, though!
Kudos to the Panhandle Health District for shutting down the meeting to address the Internet connectivity issues.

They're back in session now. Currently, a man wearing a VAXXED shirt is giving false information about mask safety.

Central District Health is holding their weekly meeting. Check out these slides: The positivity rate by week, the number of cases by week, cases by age, and the positivity rate in children. #idahocovid19
TL;DR: The positivity rate is hovering around 15 percent. While people ages 18-29 continue to make up highest percentage of positive cases, people in other age groups are catching up. Teenagers and kids also have high positivity rates.

This is not great news.
Dr. David Peterman of Primary Health, who is presenting, says there will be kids who get sick and will need to be hospitalized. He says it pretty bluntly. #idahocovid19

One reason I find this interesting is most of the #maskmandates so far have been in places where Democrats control local politics. This would be the first one in a more Republican city. #idpol #idahoCovid19 postregister.com/news/local/ida… via @thepostregister
Bonneville County’s quirky. The local GOP central committee is one of the most conservative in the state but the City Council is actually pretty liberal, it’s a mix of Republicans, Democrats and independents. They recently voted to expand the local anti-discrimination ordinance.
Mayor Casper is a Republican but a more moderate one. Bryan Smith and Doyle Beck spent a lot of money trying to unseat her in 2017. As for local GOP primary voters they’re all over. This city has produced some of the most conservative and most moderate Republicans in the #idleg

Today's general session at the Idaho Republican convention has started. Started with a prayer, pledge and the singing of the first (the one everyone knows) and less commonly known fourth verses of the Star Spangled Banner #idpol
Sen. Risch is speaking now. Said he brings greetings from Trump and that if the national media think they have him down they're wrong. "It drives the other side crazy, too bad for them. He does give America the leadership it demands." #idpol
Risch is defending Trump's handling of #COVID19. Points to the China travel ban, says Democrats called him a racist for it and the media doesn't talk about this. "Who was the first person to take dramatic action against the spread of coronavirus? It was Donald Trump!!"

Smaller crowd than the Disobey Rally, roughly 250 people at today’s protest to open up Idaho. Almost no masks, no social distancing.
There is a kind of meta aspect to the rally: lot of complaints about heavy handed tactics by police, but police are standing by while nearly every participant violates the same governor's order they say is unconstitutional. #IdahoCovid19 #Covid_19
Rep. Christy Zito (R-Hammett) is addressing the crowd. She was also at the Disobey Rally.

What a week, huh?

It's been extra stressful for @idahostatesman journalists as @mcclatchy still refuses to recognize our unanimous union. Now our election is on hold due to coronavirus shutdowns.

Here are some examples of the essential work McClatchy doesn't want to protect.
.@Jacob_Scholl, our newest Statesman reporter, is not eligible to vote for our election but will be protected by our union. We look forward to including him! In just his second week here, he broke some of the state's biggest stories, like this one: idahostatesman.com/news/coronavir…
City reporter @Hayley__Harding has had her hands full with changing directives in Boise but still managed to find time to answer reader questions about how coronavirus is affecting them: idahostatesman.com/news/coronavir…

.@GovernorLittle says he’s trying to take steps to boost healthcare capacity in a state that already has problems with access. Other states and countries are severely strained, or already at their breaking point. “We don’t think it will” happen in Idaho. #IdahoCovid19 #idpol
@GovernorLittle “It” meaning a rush on hospitals and a need for ventilators #IdahoCovid19 #idpol
@GovernorLittle .@IDHW Director Dave Jeppesen: If you don’t show symptoms, you won’t be tested. Even if we had capacity, we would not be recommending that you get tested. #idpol #IdahoCovid19
