It has been another busy sad interesting infuriating illuminating year in #globalhealth and #infectiousdisease.
Or as @DrMikeRyan told me: “2022 [was] kind of a mixed year, filled with terror and hope in equal measure.”
My colleague @SalmLaura and I asked doctors and researchers to look back at 2022 for an end-of-year episode of our @pandemiapodcast (and another episode to come soon on what 2023 may bring).
Podcast is here (in German) and a few main points to come:
viertausendhertz.de/pan52/
COVID...
of course dominated yet another year. As @DrMikeRyan told me: “I think we’ve all been a little surprised that the pandemic extended so fully through 2022.”

#Infectiousdisease encyclopedia @Darcy_ID_doc carried us through another #HIV-focused #MTC at #Hillcrest this morning!!
Chief @photon_ick & PGY3 Dr. Sheila Bhavsar shared a cool case of #Toxoplasmosis, specifically #Toxo encephalitis!! Check out our 🗝️👇!!
1. Abrupt-onset weakness needs a stroke code evaluation. Our patient presented with #fevers & < 3 hr of L-sided #weakness!! ⏱️=🧠, so this comes first!!

2. Ultimately you'll end up with a #CT scan, where you are really looking for #bleeding or other space-occupying lesions. Image
3. Our patient had contrast-enhancing lesions. #contrastenhancement = edema & mass effect. No enhancement = not space-occupying! This helps narrow our differential! Image

Hey #MedTwitter!!
This morning at our VA we had Chief @photon_ick give us a case of #ChronicSOB, where it turned out that the patient had #ABPA (#allergic bronchopulmonary #aspergillosis). It's a diagnosis we don't see too often, but one you'll definitely encounter here!! 🗝️👇
This is an #allergic condition, with 2 main predisposing conditions: #asthma & #cysticfibrosis
It can be more rarely seen in other lung disease/immunodeficiencies, but without the right lung environment like this, it is almost NEVER seen!!
There is no universally accepted criteria, but the #ISHAM criteria is often used. Let's check it out!

Traditional obligatory criteria:
- Serum #IgE RAST >0.35 OR Allergic Skin Testing (+) to A. fumigatus
- Total Serum IgE >1000 Image

1/30 In two forthcoming pieces, I examine #Japan's response to the #COVID19 #coronavirus #pandemic. I'll summarize the main non-theoretical points here (long thread alert!).
2/30 #Japan’s figures have always looked good compared to many other countries, which initially led to suspicion that the ‘real’ numbers of #COVID19 #coronavirus infections & death were being played down.
3/30 #Tokyo Metropolitan Government at least has now released full #COVID19 figures, including general death rates, seeming to put this argument to rest (but these figures are also now being challenged). japantimes.co.jp/news/2020/05/0…

A Legislature exists primarily to give checks and balances to other arms of government on behalf of the people.

We ask that @nassnigeria led by @femigbaja not to pass an #InfectiousDiseaseBill that could give individuals unchecked power over the rest of Society.
1. The #InfectiousDiseaseBill is supposed to give legal structure to public management of infectious disease outbreaks like the extant #COVID19 pandemic.

But what it does is make the head of the #NCDC and the Minister of Health powerful enough to be legal tools of Dictatorship.
2. Nigeria is supposedly a democracy and laws should be drafted with the intent to help build proper institutions that can be trusted and checked to treat people equally and fairly.

@nassnigeria should not enact laws that make gods out of men.

Although related to the Severe Acute Respiratory Syndrome (#SARSCoV) & the Middle East Respiratory Syndrome (#MERS), #COVID19 virus #SARSCoV2 shows some peculiar pathogenetic, epidemiological & clinical features which have not been completely understood to date.
#SARSCoV2 is more phylogenetically related to #SARSCoV than to #MERS. Only minor differences have been found in the #genome sequences of SARS-CoV-2 comparing with SARS-CoV. SARS-CoV-2 affinity for angiotensin converting enzyme 2 (#ACE2) receptor is higher than in SARS-CoV.

Wonderful new podcast by @CIDRAP, giving you an inside look at what my @PublicHealthUMN class w/Dr. Osterholm is like! Highly recommend the listen! Key points from ep 1 on #COVIDー19 #COVID19:
Thread 1/10
1: @mtosterholm debunks the seasonality premise. Typical in disease outbreaks to see peak infections after 6mo, this is time to understand transmission & take action. Nothing to do w/seasonality or temp, example of active MERS transmission in Arabian desert
2/10
2: Must consider long-term plans. Something between full lockdown vs no action. Find middle ground, minimize exposure in most vulnerable pop, allow transmission in less vulnerable pop b/c less severe outcomes, they become immune & continue to work in society/care for others

1/5 Beware of “back-of-the-envelope” ✉️ calculations that try to predict how many ppl will get sick or die from #COVID19 #coronavirusoutbreak , especially when they’re done by ppl who have little experience in #infectiousdisease modeling. The current situation is not that simple
2/5 Many different factors may affect the spread of the #SARSCOV2 #coronavirus . These include different assumptions about the virus characteristics (which are still emerging), how much the virus has already spread (which is unknown since widespread testing is not occurring yet),
3/5 ...how much ppl are mixing, washing their hands, & practicing other infection control measures, and what the government & other organizations are currently doing & will do in response. These all can significantly affect the spread of the #COVID19 #coronavirus

Summary of #coronavirus #COVID19 webinar by @JohnsHopkinsIH yesterday:
🔹 No country is fully prepared for an epidemic
🔹 during Ebola outbreak, 10% of tweets had bad info
🔹 Others were rumors about govt conspiracies
🔹 Others were chaos inducing tweets
1/8
🔹Propaganda blames various different populations, give rise to discrimination
🔹 There is NO CURE for #COVIDー19
🔹 There is no foolproof preventative
🔹 Surgical masks are NOT EFFECTIVE
🔹N95 #Masks are needed by medical staff. We need them to stay healthy. DON'T stockpile
2/8
🔹Community transmission - identify (testing), isolate (hospital), inform (community, health authorities)
🔹Long term care facilities have older, less heathy populations. DON'T visit them if sick. When visiting, use good #hand hygiene
🔹Facility workers should use masks
