#KetoMedStudy: Nutrient Diffs

In our study we collected a lot of dietary data, primarily to characterize diets & report adherence

In supplemental tables we reported changes in NUTRIENTS on each diet phase

In this 🧵we share the nutrient changes

Two Diet Periods for each Diet Phase

Food Delivery

Self-Provided

For each 12-week phase of the study, participants received food deliveries for the first 4 weeks to maximize adherence.

For the next 8 weeks participants bought and prepared their own food (supported by staff)

The results of the #KetoMedStudy are finally out! How low-carb do you need to go for glucose control? Is keto better than a low-carb Mediterranean diet with no refined grains (Med-Plus)? 👉 doi.org/10.1093/ajcn/n…

Here's a brief summary of the study and results:

Randomized, crossover trial of keto vs. Med-Plus (12-weeks for each diet phase) for glucose control in 33 individuals with pre-diabetes/diabetes.
Potential wins for keto:
• Lower CGM average glucose and TG with lower meds
• Lower HbA1c (primary outcome) after accounting for meds reduction
• Better outcomes with greater weight loss and higher HDL for those who started with Keto in the first phase

For past 4 wks, we've been sharing study design details of forthcoming Keto vs Mediterranean Study

This week, we want to share the 36-wk time point & get your thoughts.

Poll on next post in thread - vote on time point you think is most interesting 🤔

Which is most interesting?

Adherence best at Wk #4 of each phase (food delivered)
⬆️rigor (efficacy)

Wk #12 of each phase
⬆️generalizabilty

36 wk - reality check (effectiveness)
Who is following what?

Caveat

Primary outcome=HbA1c (ct.gov)
12wk Keto vs 12wk Med, relative to baseline

HbA1c not appropriate for assessing 4 wk change

We have Hba1c @ wks 0, 12, 24 & 36
We have CGM glucose @ wks 0, 4, 12, 16, 24

(Not enuf $$ to get CGM @ wk 36) 🙁

Most/all healthy diet patterns recommend avoiding/minimizing added sugars & refined grains, while including non-starchy vegetables

Is there additional overall health benefit to also eliminating:
*Legumes/beans
*Whole intact grains
