The UK #monkeypox outbreak is very interesting. A šŸ§µof some key points
- typically very limited human-to-human transmission, needs very close contact
- therefore, transmission is nothing like #COVID19
- to add to other #publichealth voices, very very low risks to wider general public
- if close contact, then public health teams will follow up
- monkeypox can be very nasty (case fatality rates upwards of 1%, albeit most outbreaks in areas where healthcare is v limited)
- UK has several specialist facilities for handling unusual tropical medicine cases. NHS is very good at this.
- Name is a bit misleading. First identified in monkeys, but small animals like rats/other rodents a more common host (typically in parts of West/Central Africa)

Nigeria has made a lot of progress in preparing and responding to disease outbreaks and epidemics. I remember my early days of public health; some 15yrs ago it's @WHONigeria that prepared and shared the #weeklyepidemiological report for Nigeria... a thread!

#Clap4NCDC
For every outbreak or public health emergency in Nigeria, we heard more about @WHO who appeared to be at the forefront with @Fmohnigeria and its agencies; old #NPI which merged with @NphcdaNG fought #polio with the support of @WHO @UNICEF @Rotary etc.
#Clap4NCDC 2/20
With time the #Epidemiology unit of @Fmohnigeria grew in strength and capacity to lead the response. They also produced and sent out the weekly epidemiology reports for Nigeria, which hitherto was shared by @WHONigeria in collaboration with FMOH for sure. #Clap4NCDC

Dr Aniru Conteh, an expert in treatment & research in LASSA FEVER, got the virus when he received a needle injury while drawing blood from a pregnant patient.

This day 04/04/04, we lost this hero who spent 25yrs in saving thousands of lives from LASSA FEVER.
#LassaFever #COVID19
Conteh spent his professional career working with people with Lassa fever, a viral hemorrhagic fever endemic to West Africa. Lassa fever was first brought to the attention of the public in 1969 during an outbreak in Nigeria.
Lassa fever, also known as Lassa hemorrhagic fever (LHF), is a type of viral hemorrhagic fever caused by the Lassa virus. Many of those infected by the virus do not develop symptoms. When symptoms occur they typically include fever, weakness, headaches, vomiting, and muscle pains

Increase in the no. of Lassa fever cases across the country has left many Nigerians asking why it keeps recurring

The answer is not lack of preparedness!

We are victims of our own success. Improved awareness & capacity to detect = detection of more cases

#NCDCSundayReflections ImageImage
The Lassa fever virus is spread by infected rats. These rats are found nearly everywhere in our environment and are very difficult to eliminate

With infected rats and without a vaccine, our country and several others, remain at high risk of #Lassafever transmission Image
Over the last 3 years, weā€˜ve invested in our diagnostic capacity for #Lassafever

In 2016, we had only 2 labs in #Nigeria for #Lassafever testing. Currently, there are 5 molecular labs running at full capacity & providing testing for all states in the country. 1 new lab=N100M Image

Last week, we released a public health advisory on #Lassafever

During the dry season, as a result of ecological factors, there is usually a rise in Lassa fever cases

.@NCDCgov & partners have worked hard to continuously improve preparedness & response

#NCDCSundayReflections ImageImage
With support from @FINDdx,we expanded our #Lassafever laboratories from 4 to 5

Federal Medical Centre, Owo, Ondo State, has a new lab with brand new equipment and other supplies required for #Lassafever diagnosis

Prior to this, @PandoraIDNet had supported Owo with a mobile lab ImageImage
Our #Lassafever surveillance database has been fully harmonised

This means weā€™ve established a strong surveillance system using @SORMAS_NG that has case-based data from previous years, & through which outbreaks can be reported early

Data determines all our response activities ImageImage

I am incredibly proud of our network of national public health institutes and @NCDCgov ā€˜s role in this.

Last week, we were part of @IANPHIhealth annual meeting hosted in Addis-Ababa, Ethiopia

At the beginning of the week, we were part of the new @AfricaCDC_IWD Leadership and Management Course for Directors of National Public Health Institutes in Africa.

Proud of the leadership from my brother @JNkengasong and his Team in creating a new public health order in Africa ImageImageImageImage
At @IANPHIhealth meeting, we provided feedback on @NCDCgovā€™s journey in the last 2 years, & the strong collaboration weā€˜ve built with other NPHIs- @rki_de @CDCgov @PHE_uk

Also spoke about our support to @NPHIL6 through @OoasWaho, during the 2019 #Lassafever outbreak in Liberia ImageImageImageImage

Happening now at #NFELTPX19: Presentation on Outbreak of Cerebro-Spinal Meningitis in Nigeria 2018-2019 season: epidemiological features and lessons learnt by Rimandeyati Yashe

There is a thin line between contributing to knowledge through research and prioritising public health response during outbreaks. For us at @NCDCgov, our responsibility is to find the right balance, as they are interdependent.
#NCDCSundayReflections
Both research and response are critical. We can not do one without the other. It is important for countries especially like ours, to focus on building strong foundations. Supporting communities to improve health systems that will build trust and save lives
Over the last 3 years, there has been an increased interest of global institutions- public health, research and funding institutes, in the work we do especially in response to #meningitis #Lassafever #monkeypox. We need research to develop new tools to improve response

I've had interesting and very stimulating discussions this week, that highlight the critical difference the government structure in a Federal Republic like #Nigeria creates for health security, compared to countries with unitary systems of government

#NCDCSundayReflections
Our role at @NCDCgov as included in the #NCDCAct requires us to 'support' states in outbreak preparedness and response. However, the dependence from states on the national agency is very high. Yet, we cannot ignore, because when we do, people die.
Health is on the "concurrent" list in #Nigeria, i.e responsibility of Federal & Sate govts. Unlike our security architecture which is centralised, our #healthsecurity is de-centralised. No clear line of control & we've to work hard to collaborate & cooperate across govt levels

Our attention has been drawn to a message circulating on WhatsApp and this tweet, purporting that there is a suspected #Lassafever case in the Asokoro District Hospital. This is entirely FALSE.
The recent national #Lassafever situation report indicates there has been no confirmed Lassa fever case in the FCT since February 2019. Contrary to the message being circulated, no death has been recorded from Lassa fever in the FCT this month. Available: ncdc.gov.ng/diseases/sitreā€¦
The NCDC appreciates Nigerians for bringing this rumour to our notice. We urge everyone to stay alert for #Lassafever, know the symptoms and report immediately to a health facility when symptoms are observed, to increase the chances of survival.

Managing information during outbreaks is such a critical part of our work that my Team and I never take for granted. In this era of whatsapp broadcasts, twitter threads etc, we continue to focus on strengthening our risk communications activities
#NCDCSundayReflections Image
Since we started at @NCDCgov, we have developed several ways to communicate with the public. For every suspected outbreak, we have tried to ensure that you can verify, by putting out messages within 24 hours on our social media pages/ website ImageImage
Did you know that on Tuesday every week, we publish a National Weekly Epidemiological Report? You can subscribe for this on our website and read weekly updates on Nigeria's epidemic prone diseases and where there are cases

This week, it was a great pleasure to visit Ebonyi state- the #Lassafever virology treatment centre, laboratory and the Ministry of Health. Encouraged by the progess this state has made in the last 18 months

Ebonyi is one of the three #Lassafever hotspot states in #Nigeria. In the last 2 years, the state has improved its capacity to prepare for, detect and respond to outbreaks, with support from @NCDCgov and several partners ImageImage
I was very pleased to see the expansion of parts of the treatment centre, especially the creation of more triage areas in the hospital. The treatment centre has learnt from previous healthcare workers' infections and has put in more efforts to protect staff ImageImage

A great move for health security in #Nigeria last week, as His Excellency President @MBuhari declared a state of emergency on water, sanitation and hygiene #WaSH
#NCDCSundayReflections Image
Poor water, sanitation and hygiene, is a recurrent risk factor for many infectious diseases we face in #Nigeria, especially with #Lassafever and #cholera. We simply cannot #endcholera if we do not address #WASH Image
One of the most interesting aspects of my work as head of Nigeria's NPHI is visiting communities. Across many towns and villages, we find heaps of refuse, dirty ponds, littered streets, open defecation etc. We can all do better ImageImage
