He would never be able to let OFA go if Katsuki is a vestige. Because saying goodbye to OFA would mean saying goodbye to Kacchan permanently, and he would never be able to do that. Not until the day he dies.

Here’s how I see it going down. (Don’t shit on me for this theory, I am a grieving woman) My guess is Izuku shows up, is absolutely devastated by Katsuki’s death, broken. He has a breakdown of some sort and then SFO says:
“Want me to bring him back?”

We AFO/SFO has ERI’s quirk so he can. He calls for an exchange. OFA for Katsuki. Izuku is broken and almost gives in. We see him take his hair and offer it but then whatever tf is actually going on with Katsuki is revealed. His heart starts back up OR his vestige self come in and
he stops Deku from giving SFO OFA. Smacking some sense into him. OR instead of the quirk exchange idea, it’s Deku losing his shit about to KILL Shigaraki and Katsuki comes back at that moment and stops him similar to the Final Exam, like “the one who saves people is you?”
