Now that the #Match2023 dust has settled, we have celebrated our newly matched M4s, and as @CORD_EM's #CORDAA23 gets started, I wanted to put together some thoughts on the #EM #Match2023, highlight some facts, and answer some questions. Buckle in folks, and let’s get started! 🧵
We know the drop in applicants is multifactorial, EM is about problem-solving, we're working to make the ED a better place for ourselves/our patients, & at the end of the day the majority of EM docs will still tell you how much pride we take in our work & being badass #EMdocs. 🧵
First, those who were shocked by the # of unfilled positions in the EM #Match2023 were not following the preliminary applicant data we have had since @ERASinfo released data in Oct. showing 3,023 students had applied to EM at that time (down from 3,632 in the #Match2022). 🧵

This is the first year I have not applied for #Match2023

It’s the first time I’m not worrying if I did “enough” to finally get in.

And I have to say, I haven’t felt HAPPIER or more fulfilled in years.

I had been running the #Match marathon for 4 years, kept pushing through, amping my experience, rewriting my PS, getting new LORs, working during a pandemic.

Kept ending up #unmatched, sometimes more heartbroken than the previous year, with no time to process what happened.

I had made every sacrifice to get into residency because that was the only way I saw my future as a physician

But that’s where I was wrong - I was stuck with tunnel vision of what it meant to succeed as a doctor

After #Match2022 I decided I was ending the toxic cycle

How it started...

On this day, two years ago, I was hit head on by a distracted driver on the country highway by our home. My husband died instantly and I was careflighted to the local trauma center where I remained in critical condition. It was my fourth year of medical school
Over the next few months, as I dealt with the grief of losing my husband, I struggled to overcome my injuries - internal bleeding; open fracture of the femur, tibia, and fibula; and open fracture of the 5th digit of my (dominant) left hand at the MCP and DIP joints. I worked to
independently accomplish my activities of daily living when sitting on the side of the bed, transferring myself to the bedside commode, or feeding myself were feats in itself.

Last night, I worked my first night shift as an OBGYN resident and I find myself crying in realization

Early 1900s: hospitals 🏥 were competing with each other to get the best medical students 🧑‍⚕️, and medical students were competing with each other to get the best internships offers 🤩, which were made via telegram! 2/10
By the 1940s, hospitals would make very early offers to M3 or even M2s! Students had 10 days, sometimes only 12 hours ⏳ , to respond to an offer via phone or telegram without knowing what other offers might come in! 😰 3/10

When I applied for my ERAS token in July 2021, I was in bed for almost 2 weeks because I knew what lay ahead of me. The only reason I got out of bed was to go to work at the urgent care I was with in MO or volunteer with COVID-19 vaccine clinics.

I didn't do anything but sleep..
I barely got out of bed to eat. I was just so depressed. Then, one day I realized I was wasting my time feeling like this.

I decided I was going to give #Match2022 my best shot, and if it didn't work out? I was DONE. This chapter would be over...
And I have to say, when I didn't Match on Monday and decided I was not going to #SOAP2022 I was literally relieved.

Because I had been practicing for the past 6mo in Missouri! I was not a failure, the system had failed me.

And I wasn't alone.

Out of the ~10,000 #Match2022 applicants that had to #SOAP2022

How many got ZERO interviews? It would be helpful to know % interviewed and got a spot.

Giving false hope that you might still have a solid chance is just as cruel as not Matching.
I’ll start.

4X unmatched, never got an interview in 🧼.

In main Match, got ONE interview my 3rd/4th time applying through a connection. Have only applied to IM, Peds, and FM

That’s why I decided I will not apply for residency anymore.

As we get closer and closer to #MatchDay for #match2022 I have to admit... I don't talk much publicly about NOT matching into #EM last year. It was devastating. But I think it's important that we normalize the conversation around not matching... 🧵1/11
For many, not matching is the ultimate failure coming out of #medschool. Residency placement is what we were working toward. Schools advertise on the backs of their match rate. Being left empty handed after match AND the scramble is a lonely place. You question everything. 2/11
You try for an open categorical, prelim, or transitional spot. Naturally, we're told those are the preferred options. Find a prelim surgery spot, they said—to keep your "procedure skills" up. Find a transitional year or prelim year anywhere. Get an MPH. Not tons of options. 3/11

Virtual meet-and-greets are different than in-person socials, but you should have been able to get a general sense of the vibe of the residents from each program. What are the kind of people you want to be around? How did the residents make you feel?

A letter of intent is a letter that is sent to the program you plan to rank #1 on your rank order list with the purpose of making that known to the program. Simple, right? There are some things you should know though.

My advice for #Match2022: trust your gut.

You have received an invite to a program so it means they are prepared to invest their time and energy into getting you to the next level of your career, so embrace this opportunity to evaluate the program as a fit for yourself. /1
Just as much as we are evaluating you, evaluate us. Be thorough, if you see red flags, honor those! If you feel warm and fuzzies, honor those! You (in theory) only train once, so this decision is very important - take that leap of faith. /2
In my own process, I took a massive leap of faith to get to where I am now and I have no regrets and would do it again. But in that process, I did some massive soul searching because, honestly, you couldn't have told me I was going to leave CA. /3

Thread of online interview tips for #Match2022

1) When you get your itinerary for IV, convert everything to your time zone immediately in a new document.

2) take notes about who is IV you, and you can keep that to side during zoom to reference notes on same screen.
3) To reduce zoom fatigue, change it so that only the person speaking is viewable, not the whole group.

4) Test all your equipment PRIOR to the event. Ensure your zoom & video software is completely updated.

5) if you are in noisy area, wear headphones.
6) there is features on zoom to reduce background noise and increase light.

7) be careful (slash don’t) use zoom backgrounds. People’s faces would randomly get caught off mid sentence.

Tip #1: Be thoughtful of your background.

Plain backgrounds are a fairly common choice. Pick a color that allows you to stand out (and not blend into the wall). I don’t recommend virtual backgrounds because of the associated lag and cutting out parts of your body with motion.
You can also have things in the background, but be prepared to talk about them. I had artwork with a favorite quote and a medal from my first half marathon behind me, and both came up in conversation. The rest of the wall behind me was light-colored and blank.

Getting cozy this Sunday morning watching @Inside_TheMatch! Top tips from today a🧵:
1. Submit ERAS by 09/27 to avoid web issues
2. 3-5 bullet points for each ERAS activity
3. Know all aspects of the publications you listed

4. Be specific with your hobbies. List 4-6 and expect questions during your interview!
5. Even college honors/awards count in that section.
6. Update your LOR writers during interview season, they could help get you more interviews
7. Read your MSPE, you could get questions on it during interview season!
8. If you add a language, make sure you can speak it 😉
9. There should be zero typos in your PS, make it 1 page, and avoid contractions.
10. Identify 5 priorities when applying

All right, I'm gonna level with you. This is going to be unpopular (especially with med school faculty/admin), but please know I'm speaking from experience here. Maybe this isn't universal, but here we go.
Med students heading into #Match2022 (or any match in subsequent years), please know that any and all advice from your med school administration is specifically designed to make sure *your school* is successful. They want you to match, period.
Your school does not need to worry about whether you are happy when/where/in what you match, as long as you match. You are a metric. You *not* matching is an unacceptable outcome not because it would be personally devastating to you, but because then they have to explain it.

Here’s the thing about out-of-the-box questions. You’re unlikely to get asked them, so I wouldn’t invest much time into preparing for them. However, you may get asked one or two, so it’s not a bad idea to have at least thought about them briefly so you have an answer.

Quite honestly, this might be one of my favorite interview questions, not because I enjoy answering it, but because your answer tells a lot about your values and passions, as well as what you might want your impact and legacy to be.
