Today marks one year since I was fired for explaining TN's mature minor doctrine to physicians who were vaccinating teens against #COVID19. It's been a long, disturbing, challenging year. TN continues make things hard for Tennesseans.

Since my firing, TN has continued to be near the bottom for #COVID19 vax rates overall (43rd). Among children 5-11 it's (47th) w/ 0% improvement since 6/15. Among 12-17yo, it's 49th w/ 0% improvement since 6/15. TN is among states w/ the highest counts of MIS-C.

>26,500 Tennesseans dead-- that's 1 in 257 Tennesseans. Nearly half a million cases and 42 deaths in Tennesseans under age 21. Pfizer vaccine is now FDA approved for 12+ and authorized for 6mo+. Vaccines reduce hospitalization and deaths.

2/ If passed, the bill would create a task force on maternal #mentalhealth which would identify, evaluate, make recommendations to coordinate & improve federal responses to maternal mental health conditions, create a national strategic plan for addressing mental health disorders.
3/ Part of an ongoing effort to address crisis of #maternalmortality in the US.

Each year ~700 women die during or within a year of their pregnancy d/t pregnancy-related/associated complications despite studies that show >1/2 of pregnancy-related deaths are preventable.

Finally taking a deep dive into the new @CDCgov National Vital Statistics Report on #maternalmortality in the US. This report represents the first national sample of maternal deaths gathered from a pregnancy status checkbox on death certificates. Thread.
@CDCgov According to the report, 658 women experienced maternal death in 2018, for a pregnancy-related mortality rate of 17.4 maternal deaths per 100,000 live births. This is an incomplete picture - and a gross underestimate - of the maternal mortality crisis in our country.
@CDCgov The main issues with the data come down to definitions - or who is included and who is not. While the CDC typically includes deaths through one year after the end of pregnancy, in this report they use the @WHO definition of maternal deaths - which cuts off at 42 days postpartum.

There is no excuse for the lack of organized data surrounding #MaternalMortality and its racial disparities. Black women are still 3-4x more likely to die from pregnancy complications, even when you control for socioeconomic differences. #MaternalJustice
Hard truth is that this issue is bigger than a health chart. It's about how + why the voices of black patients are discounted or denied. Not a phenomenon limited to health care by any means - a societal stain this country desperately must address. #MaternalJustice
While I appreciate state-level efforts to make #MaternalJustice a priority - CA has made great strides with inequity in maternal care, and my home state of MA is studying autopsy reports & medical charts of those who suffered maternal deaths - we must do better.

Black women are 243% more likely to die from pregnancy or childbirth related causes than white women. We can’t address solutions to #MaternalMortality without discussing solutions to the systemic racism buried deep within our nation’s health care system. #MaternalJustice
In 2014, the UN Committee on the Elimination of Racial Discrimination urged the US to eliminate racial disparities in the repro health field & collect data for #MaternalMortality - yet no systems exist. We need #MaternalJustice before more mothers fall btwn the cracks.
It's an injustice of unacceptable proportion that the richest, most powerful, most innovative nation on earth would allow skin color to impact something as profoundly universal as pregnancy and childbirth. We can - and must - do better. #MaternalJustice

1/ Public image of maternal death is a woman who, in dramatic moments immediately surrounding birth, suffers terrible tragedy

latest @CDCgov data tells different story...rather than waiting to publish academic paper, decided to share analysis right away

2/ first of all, > 80% maternal death does not occur on the day of delivery... >60% does not even occur within a week of delivery

The great majority of these deaths are at home or in the community...NOT in the hospital

3/ second, causes of maternal death shifting...deaths due to acute pregnancy conditions (hypertension, hemorrhage) have been declining for decades

meanwhile, deaths due to chronic conditions, particularly mental health on the upswing
