We've written a lot of threads on #Tennessee status at the bottom of 50 states on virtually everything from average income to life expectancy. Sadly it isn't a surprise that @TNGOP is now resorting to authoritarianism, since its members lack the skills to lead our state. 🧵
Basically the issue is that @TNGOP "leadership" puts big business/their donors first. They don't even think about average Tennesseans, since those folks don't have big bucks to pour into campaign coffers.
As a result #Tennessee has some of the worst outcomes for residents in the United States. Some of them are comparable to the undeveloped world, not a state in the most powerful country in human history.

At #NCGA #Medicaid committee, hearing from @ncdhhs that Medicaid enrollment continues to grow. Nonetheless, because of increased federal matching rate, the program finished FY 21-22 w a surplus.
@ncdhhs Lambeth: In other words, feds changed criteria, they also changed formula. Number eligible went up (many in low cost categories). At some point, we'll be left holding the bag.

Sen Hise notes there will be a "cliff at the end" will create a gap for us into the coming year.
@ncdhhs #Medicaid head Dave Richard says that's part of why @ncdhhs believes the feds will push out the end of the public health emergency to give states time to plan, adjust. Admits there will be a cliff at the end. #NCGA

And others.

"Need to effectively utilize #CNMs has never been greater," says Kohl from midwife.org

Finding a supervising provider may prove difficult.
Body of evidence has consistently shown that although these barriers do not improve care.
Kohl:
"Time to follow the economics and evidence and remove supervision of practice"

Notes that team based care is the "future of #healthcare in the US"

#medicaid presentation. We've had budget surpluses for years. Any issues could be covered.
"This is a year unlike any other in the 50 year history"
convergence of new managed care program and pandemic uncertainty.
Pandemic has increased costs for providers, the program
The good news is that the bump up in #Covid_19 spending has continued longer than we expected. Even tho there are more beneficiaries, there's more federal $$ flowing into program. Projected higher budget, but @ncdhhs had difficulty projecting by how much
Highlights of #Medicaid budget: Expanded coverage for post-partum, more waiver slots, new $$$ for home and community block grants, funding for child services reorganization, then there's IT costs. Lots going on for @ncdhhs

A few issues with the @EnergyCommerce "Build Back Better Act" provisions relating to states that rejected #ObamaCare's #MedicaidExpansion.

Based on a summary by @CenterOnBudget's @JudyCBPP: cbpp.org/research/healt….

🧵
1. Rewards Republican states for rejecting #ObamaCare.

It would authorize Exchange subsidies for people below the poverty line--but only in states that rejected the #ACA's #Medicaid expansion.

That gives GOP states a better deal than Democratic states that embraced #ObamaCare.
2. Costs more than expanding Medicaid.

Taxpayers in Democratic states would end up paying more to cover those 2 million people because:

- "private" #ObamaCare plans spend more per person than Medicaid

- governments in non-expansion states wouldn't have to contribute anything

1/ My first patient with AIDS was a young Black man - young enough to be the oldest on a pediatric ward - with wasting, diarrhea, fevers. Dx=DMAC. He died without a official diagnosis of AIDS. I was on my first clinical rotation in medical school. It was 1982.
2/ My last pt w HIV/AIDS during training was a young Black woman who presented to the ER with “asthma” that did not clear in the “asthma room” and who had PCP on bronchoscopy. She had had only one relationship.
3/ Many of my patients @GradyHealth with HIV/AIDS were Black in those early days. Some were Latinx. The myth of “gay white” AIDS has never been true, and that has cost many lives and worsened stigma.

Good morning, Missouri! Today, we all need to focus our attention on what @cody4mo and the @MissouriGOP is doing to KILL #medicaidexpansion — once again, actively defying the will of Missouri voters. Tune in: #moleg 1/ sg001-harmony.sliq.net/00325/Harmony/…
Here's how it happens: Rep. Smith, Chair of the Budget Committee, pulled medicaid expansion out of the budget and made it its own bill—HB 20. They're now planning on simply killing that single bill in committee today—actively preventing the FUNDING required to enact expansion. 2/
Let's keep in mind:
1. Expansion passed as a constitutional amendment by a majority of MO voters in 2020
2. @GovParsonMO included Medicaid Expansion funding in his proposed budget
3. Missouri is currently experiencing a budget surplus & more $ is on the way from the feds 3/

I wish people would talk about why the #ACA wasn't able to have it's full reach. That at every step of the way, it was sabotaged by people who didn't want it to be successful. (A thread) @BarackObama @JoeBiden @FutureDocs

By denying constituents #MedicaidExpansion, governors and state legislatures forced people to go w/o insurance and screenings (out of spite) In fact, they would have qualified for free Medicaid and coverage. They yelled, "See how hight your premiums are!" But go ahead, blame #ACA

Expansion will give over 230,000 Missourians health care that they desperately need. Period.

Today, I had an extended interview with St. Louis County Executive @DrSamPage about the region's response to the #coronavirus. You can listen to it on the Politically Speaking podcast feed through your iPhone/Android...OR by going to this link. news.stlpublicradio.org/post/political…
In the interview, @DrSamPage started off saying one of the biggest challenges in containing the #coronavirus is the lack of widespread testing.

"The news of the day really is we don't have enough testing. ...It completely changes how we approach this public health emergency."
.@DrSamPage says there is a "reasonable chance" that his "stay-at-home" order for St. Louis County might extend beyond it's April 22nd deadline.

Page said the high point for #coronavirus cases in the St. Louis area is likely to be in late April, though it's hard to predict.

I'm legit worried about Virginia Democrats (elected and activists) making the #ERA the goal post and not the starting line. We have both majorities and all statewide offices. This can't be the biggest ask, y'all.
Another "starting line" policy change I expect from VA Dem Majority with a big impact - eliminating the work requirements born from #MedicaidExpansion compromise. Air time on this topic is basically nonexistent.
Virginia is literally last. Dead last. Can not be lower on the list when it comes to workers' rights. So if we want to set some bold goals that are only possible with a Dem Majority let's focus on workers' rights. For example:

2/N Lots of things are happening.

The biggest one is that 2018 was so massively overpriced that 2020 (these rates) are still reflecting that problem. 2 different pricing scenarios get us to the same point with different stories
3/N 2018 was a huge policy shock for pricing with massive uncertainty on #CSR leading to insurers either running from the market entirely, retrenching or jacking up rates

As soon as CSR Terminated (October 2017), #Silverloading differentially increased silver premiums by a lot
