After a long battle, my Aunty, a Professor in Obstetrics and Gynaecology in Dhaka🇧🇩 , died of COVID-19. It’s coming up to the second anniversary of her death, and here we are again, sending healthcare workers into danger, without adequate protection.
Two critical errors are occurring again.

1 Downgraded pathogen from airborne transmission so adequate PPE won’t be available, especially in LMIC.

2 Vaccine supply and equity issues.
It is far too soon to be repeating the same mistakes again, and yet here we are.

We are asking unvaccinated, inadequately protected HCW to put their families lives on the line again… This time, the pathogen affects kids most.

A really nice study looking at the type of exposure and the type of symptoms.

Invasive = bite/scratch from prairie dog.

Non-invasive= other exposure to infected prairie dogs.

Note 80% upper respiratory tract symptoms.

People that were infected without being bitten or scratched by the infected prairie dogs showed predominantly systemic 53.3% or upper respiratory 26.7% symptoms at presentation.

In my view, this could be because virus was inhaled rather than transferred across skin.
Those that did have an obvious skin breech (bite/scratch), developed rash earlier. Some even commenced at the sit of the wound.

Those that didn’t, had constitutional symptoms earlier, rash later, without the early localising lesion to the skin to indicate how virus got in.

Just listening to @abcsydney and someone is talking about Mycobacterium leprae transmission.

They said it transmits by droplets.

This in the @NatGeo today. Statement from @WHO that #monkeypox doesn’t spread like COVID-19 (true) in tiny airborne droplets (not so true).

The problem is, if you ignore a route of transmission, no matter how minor you think it is, it will stifle contact tracing and disease control efforts.

This thing is going to do a slow but inexorable burn, probably over many years, always tending towards fitter strains.

Monkeypox is not transmitting as fast as smallpox because it’s not adapted well yet. In virus terms it’s just getting to know us. Smallpox took many, many years to become what it was.

But that doesn’t mean it is using a completely different tactics to transmit. #Aerosol
MPx is known to transmit in many ways… including aerosol in animal studies.

If I’m wrong, you wear a respirator for a couple of years and save yourself some COVID infections.

If I’m right, and we don’t act on this, it’s going to get grim.
PS. MPx has been classified as an airborne HCID since it was discovered. I’m not talking whacky science. I’m talking ordinary boring science.

It might not transmit very well, but it has been shown to transmit via this route, and some indication that aerosol transmission may cause more severe disease. Why would we risk this?
