Triggerwarnung: Sterben

We’ve said it before, and we’ll say it again: There is no shortage of nurses. The so-called “#NursingShortage” is a crisis of the health care industry’s own making.

Here’s how we got here. 👇🧵 Popular meme of Lisa Simpson giving a presentation. The proj
You might be asking, “Why would the health care industry intentionally short staff hospitals?” To make money!

To cut labor costs, and increase profits, the hospital industry deliberately refuses to staff our nation’s hospitals with enough nurses to care for patients.
This isn’t new either. For decades, the hospital industry has driven nurses away by intentionally understaffing and closing units across the hospital.

Hospitals also fail to protect nurses from infectious diseases, workplace violence, and #MoralInjury.

As of 9/22/2022 update kingcounty.gov/depts/health/c…

The vaccine I was administered & assured on site prior to injection “has no adverse reactions, not even anaphylaxis because there is no PEG” has been pulled from distribution & no longer offered. I am still disabled 18+ mths. 🧵 ImageImageImageImage
2/ I called the # from above, hoping by now, someone knows who I can contact to report adverse health event in Washington State

apps.leg.wa.gov/RCW/default.as…

But no one knows who is responsible for mass vaccination sites. It’s not publicly available info, not even lead nurses know.
3/ we know contamination existed & doses were released. When manufacturers are allowed to disconnect all information from lot numbers and no one knows who oversees mass vaccination sites or how to report adverse health events, there’s a massive problem in tracking safety signals

Take Medicine Back has sent a coalition letter to @NCAGO @JoshStein_, pres-elect of the @NatlAssnAttysGn requesting investigation into widespread violations of the prohibitions on the corporate practice of medicine in North Carolina and across states.

#PrivateEquity firms are rapidly consolidating physician staffing groups resulting in both monopolies and labor monopsonies harming both patients and physicians - placing profits above patients.

The Corporate Practice of Medicine "occurs whenever a non-physician individual or corporation exerts control over the medical decision-making or collects reimbursement for the medical services of physicians." The majority of states prohibit it @aaeminfo @AAEMRSA @AmerMedicalAssn

This is happening in U.S. & U.K. Once I got to the management & C-suite, especially in #NYC, I was unwilling to send #HCW into unsafe spaces to be injured or die.

@drbreenheroes this is the crux of the issue. “#Mentalhealth” is really #moralinjury from #ethics violated.
The frontline is disproportionately POC, women. Those making guidelines or in C-suite are able to protect themselves. Leadership manipulates data/definitions, then shames, gaslights. Non-clinicians hired to override #HCW with “opinion.” Those at frontline who speak up: fired.
Who is among these flags (& whose loved ones)? It is not those pushing out pre-prints or on TV nor those safely behind a screen from a second home. It is those at the frontline, working without support, protection, or even without pay (some hospitals asking docs to volunteer).

He spent his life as a first responder when others needed him. When he had a heart attack on the job, system overwhelm meant no resources available for him.

This is betrayal.

We’ve been asking, pleading, negotiating, screaming for this not to happen.
Those #HCW who spoke up early on #safety? Fired.

bloomberg.com/news/articles/…
Maybe listen to the ones who care enough to speak up on REAL #safety issues rather then fire them.

The current #HCW correct dismissals over refusal of #VaccineMandate claim administrators are “bullies” because that HAS been true in past.

1/9. The last 18-hrs have seen headlines appearing in print & online leading on the first paramedic on scene at the Manchester bombing not treating anyone. Many from the pre-hospital & amb world have rightly challenged these headlines, this is exactly what is supposed to happen

3/9. Even if not covered at the inquiry, a call to any number of authoritative bodies (ie. @ParamedicsUK, @BASICS_HQ, @AACE_org , @NARU_Education) wld provide context & understanding to assist in delivering an accurate & contextualised story w accurate headline.

A thread by BRD for your consideration:

Today is #ArmedForcesDay- too often a day where politicians offer platitudes about sacrifice and gratitude while they continue to send our troops to kill and die in endless wars for profit and power- not justice or freedom. #NoMoreWar /1
They praise us at their events while they gut our healthcare and sidestep accountability for the rampant sexual assault, pollution and abuses on and around our bases. #ArmedForcesDay2021 #MilitaryMeToo #EnvironmentalJustice #AuditThePentagon 2/
They pat us on the back while they line their pockets with corporate "Defense" industry campaign donations and ignore the the hunger, houselessness, and avoidable traumas our families are grappling with. #ArmedForcesDay #22aDay #MoralInjury #HousingFirst #HousingIsAHumanRight 3/

Announcement from @fordnation sank my heart today. As an ER doctor, person of color and #Scarborough native, I feel defeated 😢. The measures are not going to get wave 3 under control. #BIPOC #ERdoc @SHNcares @torontolife @JustinTrudeau @SCMirror 1/
When I landed as an #immigrant in #Canada, my life started in #Scarborough. My parents (#heros) struggled to find jobs that matched their education. That didn't stop them from working hard / hustling even if it meant 2 jobs at times to keep bills paid and food on the table. 2/
My family lived in an apt building that had mostly racialized/middle-low income families (#affordability). Not much social distancing when large crowds wait for elevators (#EssentialWorkers) or use the shared laundry facilities. This pandemic brings back those vivid memories 3/

What if the money 💰💰💰 spent by #academia & #hospitals on hiring people to spy on #SoMe, to police & shame nurses & doctors (even getting some fired for speaking up on #PPE shortage), this policing disproportionately affecting BIPOC, instead spent on hiring BIPOC & on pay gap
This is pay gap I am talking about. What kind of “#professionalism” accepts that Black women must work 7 extra months for same pay?

Professionalism on “you wore..” is used in sinister ways against Black women in particular

#blackintheivory #MedTwitter

Puritanism in #medicine, being taught to med students who are given middle author status on a publication conceived and supported by senior faculty and junior editors, is part of this harm. Many Black girls are criminalized & punished for natural hair.

I was feeling overwhelmed by all the laypress articles about #moralinjury and #burnout in frontline health care workers during #Covid_19 so I decided to do a deep dive into the literature on the mental health of workers during pandemics.

Here's what I found. #thread

1/
Seriously, so many laypress articles.

What evidence do we have on this, and further, what evidence do we have on strategies that mitigate these mental health issues in health care workers?

I presented these data the @UChicagoMed @UCCancerCenter grand rounds last week.

2/
@UChicagoMed @UCCancerCenter But first, some definitions to set the stage:

Burnout, initially described by Dr. Maslach as the prolonged physical and psychological exhaustion related to a person's work. Not specific to medicine, although the definition has been honed for medicine (see slide). #burnout

Time to resume elective surgery cases? A thread summarizing new guidance out today from the @AmCollSurgeons #COVID19

This document intro states that peak COVID rates have already been reached, or will be within a few weeks, depending on region...

[link at end of thread]
They offer four categories, each with specific issues that must be addressed locally before elective surgery may be safely resumed.

Categories:
1. COVID-19 Awareness
2. Preparedness
3. Patient Issues
4. Delivery of Safe High-Quality Care
For the 'awareness' category, the specific issues are:
✅Knowing your local COVID-19 numbers, including prevalence and incidence
✅Knowing local isolation mandates
✅Knowing your diagnostic testing availability
✅Knowing testing policies for pts and HCWs
