2. Since 2017, the fiscal situation has deteriorated to the extent that cutting civil servant salaries - the holiest of holy cows in #SouthAfrica - has entered the public debate. National Treasury is saying the cupboard is bare, but the Department of Health thinks otherwise.
3. The hole in our fiscus due to corruption and mismanagement at #ESKOM and other SOEs will continue to hit us 2026 and afterwards. We have chosen to use our limited fiscal space to fund free Higher Education not health. There are always tradeoffs, and that one has been made.

I was literally moved to tears today by the story told to me by a crying patient ... an account of her partner’s last dying week in a local state hospital ... he weighed 35kg and lay pleading for morphine for his cancer pain .... the hospital had NONE (1)
A man in the next bed had to have daily dressing changes for his recently amputated leg ... his would was still raw and dripping blood .. he’d cry in pain but the nurses didn’t give him anything and told him to shush .2
I remember a doctor who works at that hospital telling me a while back that the hospital had no morphine and yet deals with surgical and terminal patients ... and our Min of Health was sorting out a monkey problem at a nearby hospital 🙄.3

This thread is by no means comprehensive with regards to the #NHIBill but here goes.
Nationalised healthcare, otherwise known as universal healthcare, is generally bound by something called the ‘Iron Triangle’. It states that with regards to healthcare systems you can have any 2 of the following:

- Access
- Quality
- Cost effectiveness

Never all 3.
The reason for this is simple economics which acknowledges that hospitals and doctors are finite resources, and the field of healthcare an ever expanding list of treatement options and procedures. With enough money it is possible to ‘break’ the Iron Triangle but only temporarily.
