Distraught parents bring their son to you. They tell you that he has been having weakness of his left hand since 1 year. With wasting. No sensory symptoms. They have been told, Motor Neurone Disease . You look at his hand and smile reassuringly. Why?
First things first, the demographics don't fit ALS. He's a young boy. Secondly, The disease seems quite restricted. Third, no signs of upper motor neuron disease. Finally, this OBLIQUE AMYOTRPOHY is classical and clinches a diagnosis of HIRAYAMA DISEASE

Hirayama Disease typically affects young Asian males in their twenties. I have seen dozens of patients with Hirayama, never a girl (although case reports exist)
These boys tend to be lean and have long necks!
It is much more benign compared to ALS!

78 yr old with encephalopathy and seizure. MRI shows vasogenic edema within the left temporal and occipital white matter with subcortical extension. SWI shows characteristic lobar micro-hemorrhages predominantly in the effected regions.

fMRI is based on a principle called “neurovascular coupling.” This is the principle if there is increased neuronal activity in a region, there will be increased blood flow to that region to meet the increased demand

Think of it like a baby crying because it is hungry—parents immediately rush to feed it. The increased oxygen demand of the neurons immediately brings increased fuel to feed it.

For context:
🚫MRI is not routinely done for risk stratification of first-ever spontaneous ICH risk

🔮MRI is occasionally available in certain people/pts w/o ICH: may show SVD markers concerning for future ICH risk

〽️These markers are: microbleeds, superficial siderosis

The populations that this might be relevant to are:
- healthy elderly
- ischemic stroke
- memory clinic
- any pt who got a brain MRI for a different indication (e.g. migraines)
- non-ICH cerebral amyloid angiopathy (CAA)

CCA - differentiation of reactive lesions from progressive ones following cranial radiotherapy - a thread 1/n

➡️ primary & secondary brain tumors (glioblastoma/brain mets)
➡️ after high dose radiotherapy
➡️ N=33, prospective confirmatory study

➡️unclear findings on conventional MRI (psPD/RN vs PD)
➡️contrast clearance analysis with 1 h delayed CE MRI, images were substracted from each other
➡️ histopath confirmation
➡️ overall accuracy very high (84%)
➡️ subgroup of secondary tumors with high benefit (accuracy 94%!)
Therefore a potential new standard? Amino acid PET in a smaller subgroup did not outperform CCA @galldiks , perfusion/DWI more prone to institutional deviations/less potential to be standardized? @gminniti2012
