Have you ever heard of glutamate excitotoxicity? No? Well, settle in for a thread. You're going to need to know about this fascinating (and slightly scary) topic that's essential to understanding brain health. 🧵 (1/30)
Glutamate is an excitatory neurotransmitter. You need some constantly being released and taken up by neurons to keep the brain functioning. It is critical to properly function several brain pathways essential for mood and cognitive function. (2/30)
In the prefrontal cortex, glutamate signaling is necessary for executive functions like decision-making, working memory, and attention. When glutamate signaling is functioning properly, it's a very good thing! (3/30)

Thread Alert 🧵: Let's talk about Parkinson's Disease 1/25 #Parkinsons #neurotwitter
In 2005 there was this study that, although very small, showed benefits. "Unified Parkinson’s Disease Rating Scale scores improved in all five during hyperketonemia" 2/25

It wasn't designed to rule out a placebo effect. But the result should have resulted in excitement and further studies being done. 3/25

A fascinating symptom in #neurology is #Alien Hand syndrome
If anyone has seen @StanleyKubrick ‘s great film Dr. Strangelove, you might have wondered what’s wrong with the titular character

His hand has a #mind of its own!!!

#cinema #neurotwitter #MedTwitter
Simplifying , for carrying out bimanual planned movements, we need a Supplementary Motor Cortex (SMA) (left frontal lobe). This communicates to the opposite side via the corpus callosum. Lesions in any will impair proper bimanual function and cause an “alien limb”#neuroanatomy
Frontal variant (left sma lesion) : Patient will have impulsive groping and difficulty releasing objects
Affects the dominant hand
Can be due to a #stroke, #degeneration

My patient impulsively grabs my hand (with his right hand) despite me telling him not to!

A #runner who would #swing his left arm more than the right
1. I have been running outdoors for two years. Over a period of time, one gets familiar with fellow runners- specially their style, pace & endurance. This post is about one of those runners, who was a regular (like me)😆
2. He had one odd style- he would swing his right arm lesser than the left while #running or #walking.
He would continue in the same style for about 60-70 minutes of his #RUN.
His #pace was excellent- 60 minutes for a 10 km run.

3. Our interaction was limited to "Good mornings".
Six months had passed since I first saw him. At this point, I noted that I could outpace him on a few occasions (I was much slower than him to start with).
I had started to think about possibility of a #neurological #disease.

#eyes becoming smaller in evenings could be a sign of #neurological illness

1. I was seeing the last patient, on what had been a busy Monday. While counselling the patient's attendant (aged 45), I noticed that her left eye was smaller than the right.

#MedTwitter #neurotwitter
2. On enquiry, she confirmed that her eyes were of equal size in mornings, but left eye became smaller as the day progressed. This was the case for the past 4-5 months. She had no other issues. This had been noted by her family members and friends too. An eye check up was normal.
3. I discussed with her that this could be a sign of #myasthenia gravis (MG) and asked for investigations. Repetitive nerve stimulation was normal. However, acetylcholine receptor antibody was high (8; normal value<0.25). The diagnosis of MG was confirmed. CT chest was normal.

As some of you know im suffering weird #neurological symptoms atm. The last week has been pretty awful. One of the weirder symptoms is called micrographia: writing getting smaller and smaller.
↙️ My writing on Tuesday (bad not awful day)
My writing on Wednesday (awful day) ↘️ The words: corporate → research & development Location: coThe words: brief [blanked out] on extension Written in small
Micrographia in itself *can* be an early sign of conditions like #Parkinsons

In patients with Parkinsons, closing their eyes whilst writing can often help mitigate micrographia
(in my case the rapid onset followed by rapid improvement makes me think / hope its just whatever the fuck is going on with my neuro-immunology not a sign of whats to come...😬)

"#ChronicPain on the #Brain - 4 #Psychological Effects of Living With #pain"
1)
Researcg covering the different ways the brain processes pain show us that the brain reacts differently to short-term pain than it does to long-lasting pain. h-wave.com/blog/chronic-p…
2)contd....
When the body experiences the latter, it can change the #centralnervoussystem (CNS),& influence sensory, emotional,& modular circuits that would otherwise inhibit pain.
3)contind.....

Brain imaging shows we're more empathetic to others' #pain if we share skin color. Explicit bias is equal in members of majority & minority groups, but implicit bias is higher in members of the majority, an impt finding for policing & medicine. /thread onlinelibrary.wiley.com/doi/full/10.10…
The perception or imagination of someone else's #pain kicks off activity in the parts of our brains that process our own pain, #fMRI imagining reveals. This "embodied empathy" is "tuned by a variety of factors," including individuals' personalities, relationships, & experiences.
"Culturally acquired implicit attitudes & perceived similarity/familiarity" also shape our “emotional responses to others' physical #pain.”

"Considerable evidence" from #brain imaging shows we make evaluations based on "race" w/in "milliseconds even when processed subliminally."

If you really want to make a change in the world, do something hard like #nonviolent #resistance. Read up and learn #satyagraha or change. It takes #strength & #courage; physical force is forbidden even in the so-called most favorable circumstances.

If your cause is #righteous, Gandhi believed, the movement would speak for itself.

My personal justification for it is #psychological/#neurological (#CNS/#HPA).

If you use #violence, well the one with the biggest #ego or the biggest ego-extension (guns, tanks, war) will win.
Removing tools of violence (words, arms) from the equation, you form a mirror.

It's not easy though. You'd have to refine/drop/manage/elevate past your own #ego.

People - depending on personal history & psychology - sometimes become enraged at what they see in the #mirror.

Awesome study used mice + human brain organiod model to show #COVID-19 can infect the CNS/#brain, where it specifically infected radial #glia, neural stem cells + #neurons. The #virus altered the #metabolism of the neurons it infected + cld use neuron cell machinery to replicate
2/ It’s not surprising that #COVID-19 can infect neurons given the nasal cavity is connected to the #CNS via the cribriform plate, olfactory epithelium + #nerve. This led the team to state that “the brain should be considered a high-risk organ system upon #respiratory exposure”
3/ The team also showed that, at least in some #COVID-19 patients w/ #neurological symptoms, there is a robust antibody response to the #virus in the #cerebrospinal fluid (CSF). In fact, CSF-containing antiviral #antibodies blocked COVID-19 infection in brain organoids

Incredibly important hypothesis piece published today 👉 “Proposed here is a theory that #microorganisms from within the #brain and elsewhere in the body contribute to the long-term #neurological deterioration characteristic of traumatic brain injury”: sciencedirect.com/science/articl…
From paper 👉 1. “First, since the normal human #brain is not sterile, but is host to a variety of microorganisms, blows to the #skull may dislodge them from their accustomed local environments, in which they have been living in quiet equilibrium with neighboring #brain cells”
2. “2nd, upon impact commensal #microbes already resident on surfaces of the nose, mouth + eyes, and potentially harmful organisms from the environment, may gain access to the #brain through the distal ends of the olfactory + optic #nerves or even a disrupted blood-brain barrier”

Yet another team ties #neuroinflammation to persistent brain infection 👉 Here, RNA seq + western blot + antibody testing identified pegvirus inside the #brain cells of two patients who died from encephalitis (and not controls): folio.ca/u-of-a-scienti… ImageImage
Important that pegvirus acted as an intracelluar #pathogen (viral antigens were found inside glial cells + lymphocytes + astrocytes)☝️..w/ their presence in lymphocytes suggesting the “cells might serve as a Trojan horse for #viral infection of the brain parenchymal cells.” ImageImage
Also important is that detection of #pegvirus-antigen in the pre-transplantation bone marrow biopsy of one subject 👉 “indicated that this patient was infected at least 15 years before #neurological presentation, suggesting a period of viral latency.” Image
