Excited to share our publication in @CellRepMed highlighting #neutrophil activation in #PostCovid #MISC!

Led by @bpboribong and Tom LaSalle (@harvardmed), plus coauthors @D__Irimia, @EdlowLab and more

cell.com/cell-reports-m…

@MGH_RI @MGH_Mucosalists @ragoninstitute @MassCPR
Key points:
-even in mild Pedi COVID, neutrophils display IFN signaling

-Neutrophils in MISC show sepsis-like gene expression: high degranulation, TNF and IL-1 signaling

-Neutrophils in MISC *spontaneously* form #NETS intravascularly (can’t be good…)

(Read on…)
Key points (#2)

- in MISC, Spike enters the blood (from the zonulin-mediated leak of GI reservoirs) —> spike:antibody complexes trigger #NET formation.

-IgA Spike immune complexes are most inflammatory.

-addressing GI Spike leak will be important for treatment for #MISC

What do you suspect? What work up do you suggest? Details and #MayoIDQ MCQ to follow...
65M acute myeloid leukemia. Rx: CLAG-M. Prophy ACV, posaconazole, Levo, inhaled pentamidine

D14 neutropenia: fever / chills
Exam: onychomycosis, rapidly evolving lesions in arms and torso (photo)

You asked for skin biopsy. Blood culture will grow what fungus?

Case diagnosis: disseminated fusariosis

Majority got the diagnosis correctly!
Blood culture: Fusarium sp.

Rx: AmBisome / voriconazole
Hope for neutrophil recovery!!!

As the elderly are more susceptible to #COVID19 w/ signs of lung #inflammation, here's a thread about our @virusesimmunity 2016 flu paper on #antiviral resistance, excessive lung #inflammation, & a strategy to boost disease tolerance! science.sciencemag.org/content/352/62…
First we demonstrated that monos & macs from older adults have impaired type I IFN (antiviral) & #ISG responses but intact cytokine responses after #flu infection. To probe the possible in vivo consequences of weak #IFN responses in the face of robust inflammation...
... we made a mouse model that mimicked this immune response in the elderly (Mx1+/+ mice w/ genetic deletion of tlr7, mavs). Failure to induce type I IFN led to high viral titer, 2Âş bacterial infection, severe lung inflammation, and death.
