How much #nursing & #med ed curricula would need to change, today, if we all agreed that shaming folx in our care (“patients”) is never, ever ever OK?
And I include, in this, so-called #motivationalinterviewing.

Tho perhaps not its original intent, many approaches to #MI as it is currently taught assume (a) a person needs to change something about their behavior; (b) the clinician knows what type of change is best;
The way I've most often seen education on #MI operationalized represents a paternalistic approach to care that frames behavior as "choice", places culpability for health outcomes on the individual, and divorces individual behavior from structural & social forces that shape health

I totally revamped our #nursingresearch & #innovation course @UMassAmherst last fall to do just that

2/ I told the Asst Dean when I took it on that regardless of the learning objectives for the course I had only 1 GOAL:

That by the end of the semester, these students would be EXCITED!-or at very least, just a little bit CURIOUS-about a career in #nursingscience & #innovation
3/ A note about context: This was a course of ~85 Accel BS students, taught in an urban campus that is geographically ~40mins drive from main @UMassAmherst campus, in the . The classroom assigned had no windows. Class ran 8am-1030am Weds, before more courses. I say this b/c....

Tweet storm here pegging off a recent report by @edtrust on systemic racism in #highered. There’s a lot in the report, but particularly shocking is new data on the Black student loan default crisis, yes crisis, that deserves much more discussion. 1/16

@EdTrust Reported in the trade press & examined further by academics & wonks like @EduBenM is a finding first made by @rkelchen that nearly half of Black student loan borrowers default over a 12 year repayment period. Nearly 1/4th of Black bachelor degree holders default. Horrible. 2/16

I dove into the FEC data, and it's pretty clear:

Grassroots health professional donors far prefer Sanders and Warren to Biden and Buttigieg. 1/
For doctors, donations went mostly to Warren and Sanders, with Sanders slightly receiving more.

Buttigieg received a substantial portion.

Biden received very little.

Aside from the commitment to 50,000 more nurses that has been heavily commented on, plans include:
1. £5-8k annual maintenance grants for students
2. 14,000 new undergraduate nursing students

3. 5,000 new nursing degree apprenticeships
4. 7,500 extra nursing associates
5. Increase NHS funding by 29% by 2023
6. No reduction of tuition fees but focus on interest rates to reduce burden on students

First of all, what is it? The exact mechanism is unknown. What we do know is that there is calcium in the arterioles of the skin, with arterial thrombosis. This interruption of blood flow causes painful ulcers and retiform purpura. Remember this?

That interruption of blood flow causes the clinical picture of calciphylaxis - retiform purpura with a predilection for fatty areas, violaceous borders, necrosis with ulceration, and TERRIBLE PAIN. Without the pain, I really think one needs to reconsider the diagnosis!

2. Successful change & innovation requires all professionals to be valued & enabled to practice at the top end of the skill set. Structures & culture within #NHS sometimes minimise this.

Nursing, a thread:

Saturday- my kid and his family kicked my butt, wore me out, had me frustrated, emotional, overwhelmed.

Sometimes you advocate so hard, and people listen and don’t listen and challenge you and you leave exhausted.

I didn’t want to do it again on Sunday.
Sunday- I have the same kid back.
He’s sedated/intubated

I don’t expect many changes for the day, but the team had other plans so we end up cutting sedation & preparing for extubation.

These are all good signs. The family is excited. The patient is scared
I get the family to leave the room, and the patient nods and tears up when I ask him about his worries.

Seeing absolute fear in a child’s eyes pulls your heart harder than anything else. I hold his hand, pray with him, we listen to his favorite music, & put on a brave face.
