Disheartening things I find w/ #ChronicPain & #ChronicIllness is misunderstanding &judgement from others. Weight seems to be a big thing for others. Overweight or thin seems to be an issue. Is it though or are those "others" basing it from their reality instead of the reality of

I have been working with woman who has been struggling with a number of #physicians & getting mixed messages re #prescribed #medication. She was pulled off a combo of meds that worked for her a few years ago. Since then has 1
struggled with accessing meds that worked & #stabilization of health conditions. In desperation to improve QoL reached out to #Clinic above. They prescribed w no issues. It was difficult for #patient to access at #Pharmacy. She then attempted to contact #physician re questions 2
& was told could not talk to #physician & then received this message last night. I am furious & looking for advice on how she should disengage & get back w regular #physicians, yet she is afraid they will terminate care based on her actions & she is fearful she did something 3 Image

Almost 25 years ago I entered the world 🌍 of #raredisease when 1st my husband was dx w #PRSS1 & then our children.
What I’ve learned is #HealthcareInAmerica is brutal & often #harms those most in need.
The system is broken.
There is no disguising it.

Also, at midcareer female success is threatening to men our age AND to younger men who rely on patriarchal patronage systems. Women, overpoliced by the system, then, yes, are tougher bosses AND we *do* tend to adhere more closely to ethics.

I do do all:
On #MedTwitter there is a pattern of “callouts”, yes vs all genders & ages, but also pattern of misogyny and misogynoir. It happens towards nurses & doctors alike. One of the reasons I connected with @ShrimpBrokkoli is because I saw a yt male resident ridiculing her online.
I saw that behavior being taught by NON-clinical faculty & that REALLY crossed a line.
If you are a woman + nurse + frontline + Black/WOC, you already live with so much violence towards you. Added online made up drama from a trainee vs nurse?

This is what targeted racial harassment looks like: nitpicking things like “smiled” or “wore clothes like a celebrity, as a celebrity” or “bought cookware.” It is part of the “gold digger” narrative.
This is common in #healthcare#professionalism” where there is hazing & bullying by racist &/or classist made up non-rules lacking standards…yet people are called “breaking the rules.”

It’s exactly how bullies operate: double standards, mislabel/smear, exclude, marginalize.
In the U.K., “BAME” includes the formerly colonized, thus making race constructs or power different from U.S. as latter had race-based chattel slavery.

Thing is, those formerly colonized have many who have internalized self hate & racism.

Now @OptumRx & @SanfordHealth #insurance sending out letters re #opioid #pain #medications. My son had to move from a #Sanford clinic to another clinic due to #abandonment. Yep, abandonment. Plain & simple what you did was harmful & not based on #patients needs. You need 2

Um absolutely adore this thread! I must read for #NEISvoid #CPP #RareDisease. My kids experienced much of what is mentioned in thread. 1st they have a rare disease, 2nd had a rare medical procedure to try to address symptoms, it caused complications. My kids now have T3 1/

I wanted to share some comments by #parents whose children have #pancreatitis. As I have shared before, the #pain care for these kids is NOT adequate. Their #pain is often not being addressed. I have asked parents to start writing up their stories to share. When #pain is not 1/
addressed it causes trauma, not only for the child/youth but parents. We know unaddressed #pain can lead to #chronicpain. I absolutely believe in #biopyschosocial approach but what we are seeing is a #psychosocial approach being utilized and #bio aspect missing. This leaves 2/

In light of the Boston police Union leader child abuse, this is very plausible, even probable. I have 2x, on behalf of someone else, reported, via Title IX - the woman or gay man was not feeling safe enuf to report. Yes, there is blowback when one reports. But we can’t be silent.
I am a mandated reporter as a #pediatrician. I *must* report for my patients or face legal consequences. Am same for staff, students, trainees. But in #EmergencyMedicine who do I call if a child may be unsafe at home? Sometimes, cops. Are cops safe?

Further, suboxone has required special training and certification, adding an administrative barrier and creating an added fear barrier among risk-averse clinicians whereas oxycodone can be prescribed easily with the DEA certificate we all have.
Because doctors/prescribers are definitely lacking in administrative burden and CME obligations, we're just twiddling our thumbs eager for someone to ask us to get MORE educated, taking time off from patient care to get this:

The law enforcement side sees doctors through this lens. So doctors seek to limit their risk and liability. I'm all for taking down those who harm patients. I've testified in cases like the ones below for the AG's office as a medical expert.
